  • A short descriptions of the Kelvin TOP-SET process

  • This should be a clear, relevant, accurate and brief description of the incident. It may include labelled diagrams and photographs. Write in simple jargon-free language, and assume your audience has little or no technical knowledge. Number your paragraphs and pages for easy reference, and don't use bullet points for long lists.

7) Timeline

  • Include the step by step sequence of events with timings

8) Findings

  • Discuss what has been found without repeating what has been given already.

9) Root Cause Analysis

  • Use the Kelvin TOP-SET system

10) Recommendations

  • State what is recommended to avoid recurrence and make improvements. Use SMART actions.

  • A brief discussion of what has been established and recommended.

  • List all technical terms, abbreviations and acronyms in alphabetic order, along with their meanings (e.g. PTW - Permit to Work).
