An incident report, also known as an incident statement, is a documentation of an event, condition, or situation in which have caused injury to a person or damage to a company asset.
Incident reports provide the organization insights on what areas in the business needs to be optimized in order to prevent recurrence of injuries and damages in the workplace.
As an outline, an incident report should contain the following key elements: general information such as location of the incident, determine whether the incident resulted in any damage to people or property, include the names of the people affected, record names of witnesses, and documenty corrective actions
It is the responsibility of the immediate supervisor or person responsible for the work area to investigate and file the report.
