15 Fundamentals Assessment

  • Employees and subcontractors truly believe they have the right, and the responsibility to stop any task or activity deemed to be unsafe.

  • There is evidence/ examples of recent interventions conducted by line management on unsafe acts or unsafe conditions

  • TOFS is actively practiced at rig site

  • The TRA process is well understood by Managers and Supervisors including risk rating using the Risk Matrix

  • Task Risk Assesssments are routinely used to assess risks associated with the tasks at hand

  • TRA results are discussed at pre tour/ pre task meetings

  • Pre job discussions are held by Supervisors whenever changes occur in planned activities

  • A list of activities subjected to Permit To Work is visibly posted and understood

  • PTW are systematically requested for confined space entry, working on energized systems, or conducting hot work activities

  • All PTW are formally authorized by the Person In Charge

  • Lock Out -Tag Out equipment such as padlocks and tags is readily available at the rig site

  • A Confined Space Entry Plan is prepared for any confined space entry

  • Lifting equipment and gear in use at the rig site is certified and routinely inspected

  • Non-conductive Tag lines are systematically used to guide overhead loads

  • All Personnel operating lifting equipment such as cranes, forflifts and winches is adequately trained

  • Only trained personnel is authorized to work at height above 6 feet 1.8 M

  • Adequate fall protection devices are readily available and used when working above 6 feet

  • Working alone is always undertaken with a designated “working alone” contact.

  • Drivers of company vehicles are properly licensed, trained, and have suitable experience for the vehicle they are assigned.

  • Trip plans are systematically prepared prior to the commencement of any journey

  • Drivers and passengers are wearing seat belts at all times

  • Drugs & Alcohol tests are conducted at the rig site

  • Required PPE (generic and specific) is available and used by Saxon personnel and subcontractors at the rig site

  • Incident and near incident reporting requirements are well understood by managers supervisorsand employees

  • Recent HSE incidents have all be reported reported to management within 4 hours

  • Employees are trained and experienced for the task at hand or work under direct and permanent control of a supervisor

  • Emergency Response Plans are defined for applicable incident scenarios and understood by employees, contractors, and customers

  • Regular drills will be conducted to ensure a smooth and suitable response to all emergencies that may arise.
