Does the lab have a clean and orderly appearance? (Good housekeeping, no blocked fire alarms/extinguishers/sprinklers/electrical panels/showers/eyewash; no food or drink)?

Is all lab safety equipment in good working condition and tested and documented in accordance with EHS requirements? Biosafety cabinet, eyewash, shower, fume hood, etc.

Have all personnel in the lab, including the PI, completed their relevant EHS trainings (Laboratory and Research Safety, Laser, Biosafety, Radiation, etc.) (initial/refresher) and are the training records available for review?

Has the lab completed the Unit Specific Plan, and has it been updated, reviewed, and signed by all personnel working in the lab within the past year?

Has the annual Lab Self-Inspection been performed?

Is PPE and general attire appropriate and present in the lab, and do the lab personnel use it?

Are containers of hazardous materials (chemicals, biologicals, and radioactive) properly labeled, in good condition, and stored properly?

Are chemical wastes managed properly? (closed, labeled, secondary containment, not leaking, segregated, inspected, and signage)?

Are biological wastes managed properly? (labeled, red/orange bag, not leaking)

Are radioactive wastes managed properly? (closed, labeled, secondary containment, not leaking, segregated, inspected, and signage)
