  • **Does Truck Binder have all required and CURRENT Insurance Card, Annual Inspection and Registration and all other DOT paperwork?

  • **Is Fire Extinguisher secured with correct sized bracket?

  • **Is Fire Extinguisher current on inspection tag current and charged?

  • **Are all seat belts functioning with no tears and has the ability to latch properly?

  • **Is cab free of winch bar or other heavy items?<br>

  • **Are all head lights, tail lights and marker lights in working condition, not cracked or broken? Zero tolerance ONLY if brake lights or turn signals do not operate. All locations must move to LED lighting on equipment.<br>

  • **Triangles - Does each vehicle have 3 per truck, secured in the cab or tool box or in the trailer tool box on tractors. Cannot be broken or missing parts<br>

  • Windshield - is windshield free of cracks, stars, chips and glazing? (in drivers line of vision).

  • Does horn(s) operate as required?

  • Is unit free from any significant Body Damage? "Note fiberglass, mirror, body or ICC bumper damage".<br>

  • Are mirrors free of cracks, undamaged brackets and secured to trucks door or fender?

  • Are all engine fluids at required levels?

  • Does fifth wheel have mounting fasteners as required? Is the fifth wheel plate free of damage and cracks?<br>

  • Is unit free of audible air leaks?<br>

  • Are tire rims and seals free from leaking?<br>

  • **Tires - Do all tires have at least 4/32 tread and free of sidewall damage cords are in tact?<br>

  • Are mudflaps on both sides plain black or US LBM Logo'd? Cannot be torn and hangers can't be damaged. <br>

  • Do tractors have reflective tape on brackets?

  • Reflective Tape - Is 1/2 the length of the trailer (i.e., a 45' trailer needs 23' of tape on each side) and across the rear of the trailer and/or located on mud flap brackets? Is reflective tape on the back of all box trucks?<br>

  • **Straps - Are straps free from cuts over 1/2" and free of frayed areas at hooks, or stitching coming out? <br>

  • Tool Boxes - Do all doors and latches functional and free from damage?<br>

  • Does vehicle have a readable "How Am I Driving Decals" on rear of Princeton's and Box Trucks?<br>

  • Does vehicle have GPS or video sticker?

  • Header Boards -Do all trailers have header boards attached? If not, place trailer out of service until header board can be installed.

  • Is tire size OEM standard? (Cannot have upgraded tires, i.e. lifted mud tires)

  • Does unit have a "How's My Driving" Decal?<br>

  • Is the unit free of any unauthorized bumper stickers, decals or window shades?

  • Are tires above 4/32 and undamaged?

  • Does tailgate latch properly open/close with all trim parts attached?

  • Forklift
  • (2) points will be awarded for each item in compliance. **No Tolerance Items - Operators observed not wearing seatbelts, any expired forklift license, non working parking break or any visual obstruction on the overhead guard. If not in compliance, a score of (-50) will be scored under the no tolerance section.**

  • Is unit free from excessive hydraulic oil leaks or oil pooling under the forklift or on access steps?

Fleet Technology and Maintenance

  • Are all drivers logging into ELD system when driving vehicles as required? This is to include non-CDL drivers.

  • Has engine be set to idle less than 10 minutes? Review any idle alerts from Telematics reports.

  • Are CDL Drivers utilizing HOS as required by FMCSA?

  • Are defects noted by the driver in the DVIRs being closed/corrected within ELD Flex as required?

  • Are all drivers using the BisTrack delivery app to capture pictures and signatures?

  • Are Dispatchers adding drivers name to journey planner?

  • Is location utilizing Holman for all Maintenance, PM and parts? Review AMEX, petty cash and any other purchasing options to confirm no outside purchases have been completed.

  • Is location current on all equipment maintenance through Holman? Review Holman reports for any missing or overdue for PM Service units.

  • Pickup Truck/Forklifts/PB's - Is the working month of DVIRs displayed on the corresponding clipboard?

  • Does the maintenance file contain 90 days of DVIRs? There must be one inspection for each shift. EACH operator must inspect EACH forklift he/she operates.

  • Drywall Boom Are daily inspections being completed and placed on clip board system?

  • Is the drywall boom OSHA certified annually as required by OSHA? -Check equipment file

  • Piggybacks -Are PB's being inspected daily as required by OSHA?

Driver Qualification Files

  • **No Tolerance itemson expired medical or drivers license**. Allowing a driver to operate equipment on expired documents will result in a -(50) point deduction under the no tolerance section**.

  • Is location following all CDL protocols when hiring?

  • Has location provided JJ Keller with all necessary items for the Driver Qualification File (DQ File)?

  • **Do all CDL drivers have a unexpired medical card and license on file with JJ Keller?

  • Is the driver handbook acknowledgment in HR Personnel file?

BisTrack Delivery

  • Not adding driver(s) to Bistrack dispatch delivery will result in a -(50) point deduction.

  • Is location adding all drivers to journey planner as required?

Holman Insights

  • Any service outside of Holman or missing data will result in a score of (-50) entered.

  • Location current in Holman for all maintenance, PM and repair expenses, annual DOT inspection, expiring license plate dates or missing tag numbers will result in a -(50) point deduction.

Pickup

  • Missing current documentation. If not in compliance, a score of (-50) will be entered.

  • Were violations discovered while completing audit?

Truck(s)

  • Negative roadside inspections resulting in an OOS from the last fleet audit will have a -(50) point deduction under the no tolerance session

Fleet Technology

  • Are there unassigned engine hours/miles greater than 1% of six month total miles?

  • Are all drivers logged into HOS as required?

  • Are all drivers completely a daily pre/post trip in ELD Flex?
