Have arrangements for welfare of the workers on site been verified? (toilets, potable water, hand washing, clean area for breaks and eating)<br>

Are ABB OHS requirements agreed by contractor or subcontractor in writing? <br>

Are all field engineers trained in First Aid/CPR?

Are working hours properly controlled to prevent fatigue and is travelling to and from the site considered?<br>

Is Risk Assessment(RA), Job Hazard Analysis(JHA) or Job Safety Analysis(JSEA) done properly ? <br>

Are hazards and near miss incidents reported properly?<br>

Do Service Managers conduct Safety Observation Tours (SOT's) regularly as required?<br>

Are arc-flash risk assessments that includes incident energy levels, arc-flash boundary calculations, and PPE category ratings availble before the job starts? If not available, has the ABB Matrix for arc-flash PPE been considered and evaluated?<br>

Is arc-rated clothing (8cal/cm2 long sleeved shirts and pants or coveralls) worn as standard uniform? <br>

Are workers provided with the proper PPE including Electrical Certified Boots (EH) and Voltage-Rated Gloves for those who are exposed to electrical risks?<br>

Is dielectrically testing of voltage-rated gloves and sleeves performed at required intervals? (Every 6 months)<br>

Do Service Engineers who are exposed to electrical risks have ready access to a voltage detector or device that is rated category III or higher and are these devices calibrated annually?<br>

Are there any hazardous wastes from ABB activities at this worksite ? If "yes", Are hazardous wastes stored at appointed place or disposed by a licensed vendor?<br>

Is hot work done safely? (fire extinguisher, away from flammables/combustibles, safety watch, hot work permits...)

Are electrical tools and equipment were inspected and used on site safe? (i.e. no defective tools used, GFCI, RCD, or ELCB utilized for all temporary power supplies)<br>

Is all relevant electrical work performed safely? (7 Steps followed, approach distances identified and demarked, trained and competent, PTW, lockout/tagout, watchmen etc.)<br>

Is working at heights operations done safely and is equipment properly inspected? (full body harness, proper working platform, mobile elevated work platform, proper ladder or scaffold, etc.)<br>

Is crane operation safe? (Trained, inspected pre-use and annually, tagline used, clear of people, proper signaling, overhead lines considered and delineated, etc.)<br>

Is there any confined space operations done safely? (i.e. entry permit, atmospheric monitoring, attendant, rescue plan, communication, etc.)<br>

Are testing/commissioning activities carried out properly (i.e test area defined, warning lights, barriers, risk assessment, competent persons)?<br>

Does the site supervisor/manager check the working conditions and supervise the behavior of contractor employees?<br>

Are adequate competent HSE Advisers allocated to the site?<br>

Contractors have been instructed and trained in the site risks and hazards and understand and are implementing any necessary control measures?<br>

Evidence of competence of contractors for any high risk activities (i.e. electrical work) is present on site?<br>

Does PICW ensure that HSE compliance to ABB minimum standards are adhered?

Is there a prequalification process for all contractors that includes checking safety programs, injury history, training and competency, insurance liability policy and indemnity, government inspections, fines and penalties, etc.?<br>

A process of audit/inspection is in place to check contractors arrangements and compliance to ABB Standards?<br>

Contractors have been provided with ABB minimum HSE requirements?<br>

Is contractor performance evaluated on an annual basis?<br>
