Documentation

  • Are all previous audit actions completed or open but still within due date?

  • Any viewed pre use checks have been completed satisfactorily (note any issues/ defects from pre use inspections)

  • Are checked reports up to date and accurate i.e. shift reports

  • Are checked documents being used current i.e. within review dates and filled in correctly

  • Is there a skills matrix available to view and is it current?

  • Is the site OCD tool up to date i.e. no items past due date

  • Does the OCD tool reflect the current staffing at site?

  • Are there documents to support the OCD tool and staffing are aligned i.e. training docs, EOR forms etc.. Look for at least 2 pieces of evidence

Housekeeping

  • Condition of office / workstations?

  • Condition of checked vehicles used at site - internal / external?

  • Condition of any other checked Axil related areas on site?

  • Condition of walkways, vehicle routes, fire exits, car parks

  • Are cleaning utensils available to use and in good working order?

  • Is a 5s culture being adopted and adhered to?

Waste

  • Are checked waste storage containers in use correctly labelled or colour coded? (Items not in use i.e. spares do not count)

  • Are the checked waste storage containers in a good state of repair?

  • Are any checked waste streams being stored and segregated correctly

  • Are any checked spill kits clearly signed and adequately stocked?

PPE

  • Are the checked Axil personnel wearing the correct PPE for site (including agency staff) ?

  • Is the checked PPE clean and serviceable?

Health, safety and environmental

  • During the visit were there any health, safety, or environmental risks or issues observed?

  • Are the checked safety signs/ notifications clear and legible? (Around the Axil working area/site as well as on machinery)

  • Are there adequate provisions in place for First Aid on site? i.e. staff trained, first aid supplies

  • Are First Aid emergency instructions clearly displayed? i.e. contact numbers, what to do

  • Are fire extinguishers correctly placed, clearly marked and within inspection date?

  • Is the lighting sufficient in work areas? (has a lux reading been taken?)

  • Are any applicable barriers, fencing, gates and handrails in a good state of repair and working correctly?

  • Are any applicable storage cabinets/ racking at site labelled correctly and in a good state of repair?

  • Is PAT testing in place and in date?

Behavioural observations

  • Eyes On Path (watching where you're going and go where you're watching - looking for/being aware of hazards)

  • Eyes On Hands (aware of hand placement - watching where hands are placed during work or near hazard, not being distracted)

  • The worker is positioning his/ her body to avoid injury by any moving hazards (caught between, falling objects)

  • Lifting (correct lifting techniques - using legs, back straight, weight close to the body, feet flat on the ground, knees bent)

  • Ascending/Descending (proper climbing techniques on trucks and ladders/ stairs - facing the ladder, not skipping rungs or steps, hold onto handles)

  • Travel Path (non-hazardous route of travel, a path of the least potential incident - walking/driving around uneven surfaces, rough terrain or anything in your path)

  • Travel Speed (rate of speed to minimize potential incidents based on environment, surroundings or changing conditions)
