Checklist

General Guidelines

  • *Spend approximately 15 minutes touring area to view items listed below.

  • *Total Observation process should take 20 minutes from time of pickup to turn.

  • *Mark appropriate items by choosing either Yes or No. Mark N/A if item is not applicable.

  • *Give/ask feedback/results to available people using positive reinforcement.

Housekeeping

  • 1. Aisles established and clear.

  • 2. No tripping hazards present.

  • 3. Garbage cans not overflowing & in designated location.

  • 4. Carpets and rugs secure.

  • 5. Floors kept dry - not slippery.

  • 6. Entrance mats available (wet weather).

  • 7. Good housekeeping practiced - excess paper, computer cartons and trash removed.

Tools & Equipment

  • 8. Wall shelves designed for intended load.

  • 9. Heavy storage cabinets, bookcases and file cabinets secured/bolted from tipping.

  • 10. File drawers closed when not in use (only one open at a time to prevent tipping).

  • 11. Chairs in good condition and adjustable (when appropriate).

  • 12. Paper cutter equipped with guard - blade spring functioning.

  • 13. Step stools available for use, where needed.

Body Use (Ergonomics)

  • 14. Seated - body in good position (neutral neck, legs & elbows @ 90 degrees, feet flat on floor/foot rest).

  • 15. Standing - body in good position (neutral neck, elbows @ 90 degrees, feet flat on floor or 1 on foot rest).

  • 16. Chair - at comfortable height & positioning.

  • 17. Keyboard - elbows close to body, wrists in neutral position.

  • 18. Mouse - wrists in straight & neutral position.

Maintenance (incl electrical & fire)

  • 19. All extension cords are 3-wire type, no longer than 10' in length, and in good condition-no splices or broken insulation.

  • 20. Only one extension cord in use (not daisy chained).

  • 21. Equipment power cords in good condition.

  • 22. Wall outlet in good condition and junction box covers in place.

  • 23. Electric circuit panels clear (at least 30 inches open area).

  • 24. Power strips equipped with overload protection.

  • 25. No wires or extension cords under carpets or rugs, through doorways, or placed in other traffic areas.

  • 26. Fire extinguishers have current inspection tags.

  • 27. Exits not obstructed, properly marked and kept unlocked during normal business hours.

  • 28. Office has current emergency action plan posted.

  • 29. Medicine cabinets have been stocked and initialed by vendor.

Stairways, Corridors, Storage Areas

  • 30. Adequate lighting in place (including emergency lighting).

  • 31. Stair carpet/treads in good condition.

  • 32. Stairways clear - not used for storage.

  • 33. Corridors kept clear of equipment.

  • 34. No storage within 18 inches of sprinkler heads (24 inches of ceiling where no sprinkler system exists).
