  • Is the mobile equipment on site in good operating condition?

  • Has the company provided proof of the operator's training?

  • Is there a first aid kit on site?

Trenching

  • Are there any trenches over 4 feet deep?

  • Is there a competent person onsite for daily trench inspections?

  • Has appropriate measures been taken to prevent a trench collapse?

  • Are the protective measures installed correctly (box, jacks, slide rail, etc)?

  • Is the trench trench properly benched or sloped?

Fall Protection

  • Are employees using proper/correct fall protection measures?

  • Is the equipment installed properly?

  • If observed/inspected, is the equipment in good condition?

  • Are the guardrails properly installed?

  • Is there any open holes not properly covered?

Scaffolding

  • Has a competent person inspected the scaffold?

  • Has the scaffold been erected within OSHA regulations?

  • Are the guardrails and endrails in place?
