HAZCOM

  • 1. Are containers labeled in accordance with the HAZCOM standard?

  • 2. Are chemicals segregated by hazard and stored properly?

USP Hazard Assessments

  • 1. Have hazard assessments been conducted for activities where hazards exist?

  • 2. Have the hazard assessments been documented/retained?

  • 3. Have the hazard assessments and lab safety practices been communicated to all laboratory personnel?

  • 4. Have all personnel in lab (including PI) received initial Laboratory Safety training and/or refresher training?

  • 5. Have SOPs been created for hazardous activities?

PPE

  • 1. Are eye protections appropriate for tasks (safety glasses, goggles, face shields, etc.)?

  • 2. Are substantial shoes worn with no open toes or sandals?

  • 3. Are gloves selected and worn according to the hazard?

  • 4. Is protective clothing worn while working at benches when appropriate?

  • 5. Is the PPE sufficient to address the hazards?

Machinery

  • 1. Are machines adequately guarded?

  • 2. Are electrical panels free of obstructions?

  • 3. Are electrical and extension cords in good condition and free of splices?

  • 4. Has laser-containing equipment been registered with EHS (if applicable) and inspected?

  • 5. Have equipment users completed required trainings for machinery, lasers, etc.?
