Vehicle Readiness

  • Is van exterior undamaged, clean with C&W Services graphics in place?

  • Are ladder racks secured with a padlock and are ladders secured properly?

  • Is van interior clean, organized with supplies & equipment secured?

  • Does the van have safety cones and emergency road side triangles?<br>

  • Is the EFM Mobile Maintenance handbook available and up to date?

  • Does the handbook contain contact information in case of an emergency, including the number to the Medcor Triage Nurse in case of an injury?

  • Does the van have a first aid kit on board and is it properly stocked?

  • Does the van have a fire extinguisher on board and is it easily accessible?

  • Are vehicle inspections being performed at each fill-up?

  • Has the employee read and understand the rules of our Vehicle policy?

  • Has a MVR checks been conducted on the driver in the past year?

Safety & Environmental Readiness

  • Does the technician understand the Cardinal Rules and can he provide a few examples of these rules?

  • Does the technician understand C&W Services Safety Pledge and can he provide an example of what it means?

  • Does the technician understand his roles and responsibilities as it relates to injury & incident management?

  • Does the technician understand the PPE Policy and does he have the available PPE in stock?<br>

  • What was the previous or most recent safety training received? What did you learn from the training and what have you done to prevent injury?

  • Does the technician have the necessary universal waste recycle packs and is recycling per environmental guidelines?

  • Is the technician keeping refrigeration logs as required by the EPA?

PPE Readiness

  • Are hard hats available and in use?

  • Is eye and face protection equipment available and in use?

  • Is hearing protection available and in use were required?

  • Is there hand & arm protection available and is use?

  • Is the technician wearing the required foot protection?

  • Is there fall protection equipment available and in use? Required when falls of 6' or greater

Tools and Equipment

  • Does the tool stock appear to be adequate for the work that is being performed? Review Tool inventory!

  • Are tools & equipment in good conditions and safe for use?

  • Is the technician using the correct tools & equipment for the task being performed?

  • Are tools & equipment inspected before each use? Power Equipment, Ladders, Aerial Equipment, Utility Vehicles, etc.?

  • Does technician have access to both mobile devices (iPhone & iPad) and is utilizing the applications correctly?

Work Area Preparation Readiness

  • Permit to Work Guideline: Did you check in with the building owner or requestor before starting the work?

  • PPE Guidelines: Do you have the correct PPE for the task you are performing?

  • Electrical Work Guidelines: Are you trained and authorized to perform electrical work?

  • LOTO Guidelines: Are you trained and authorized to perform LOTO? Have technician demonstrate or explain a couple of scenario's

  • Hot Work Guideline: When hot work is performed are you following the hot work permit instructions? Are you posting the permit as required? Show me your completed hot work permits.

  • Spills, Trips & Falls Guidelines: Issues have been identified have been corrected?

  • Portable Ladder Guidelines: Is the correct ladder being used for the job and are they inspected before each use?

  • Lifting and Hoisting Operations Guidelines: Does the technician review these guidelines before each lifting operation?

  • Work at Height Guidelines: Is the technician using the proper fall protection and is he authorized to use aerial equipment?

Know: Acquire and keep up to date knowledge or the employee task

  • Can you please explain to me the job task you are performing now?

  • What other task do you normally perform?

Understand: Understand the general hazards associated with operations of C&W Services

  • Are there any safety hazards with this type of activity/task?

  • Is there anything causing you to rush or become frustrated on this work or other work?

Controls: Consider information regarding incidents, hazards & risk and respond in a timely way to that information

  • Are you aware of any injuries from this or similar injuries?

  • If yes, were any changes identified or made to prevent future injuries?

  • Is there anything you can think of that would make this job easier or safer?

  • What do you like about our companies safety programs?

Hazard Reporting: Hearing about hazards before they result in injuries builds a true preventative safety management program?

  • Have you noticed any safety hazards (a good catch) or been involved in or witnessed any near-miss incidents?

  • Do employees talk to each other and/or their supervisor/manager about safety hazards and near misses? If not, why not?
