  • Have all personnel involved in the task read and signed onto SWMS?

  • Space isolated from introduction of any substance, condition, activation or energising by plant or services?

  • Is there a ‘Confined Space Entry Permit’ be in place prior to work commencing?<br>

  • Are all personnel entering the confined space signing in and signing out?

  • Are all personnel conducting confined space work trained and competent?

  • Is there a sentry in place and is the sentry trained in Confined Space Entry?

  • Is there communications in place, suitable and understood by employees?

  • Signs must identify the confined space and be prominently located next to each entry to the space?

  • Signs must inform workers they must not enter the space unless they have a confined space entry permit?

  • Is there a safe access and egress for the confined space?

  • Has the confined space atmosphere been checked before entry and continually (see permit)?

  • Is there a calibration certificate for gas testing instruments available?

  • Is there sufficient lighting in the confined space for workers to conduct the task safely?

  • Is equipment positioned so no exhaust fumes are able to enter the Confined Space and ventilation is adequate?

  • Is there a rescue plan that addresses rescue and retrieval of personnel from a confined space?

  • Has the rescue plan been discussed among the work crew before entry (check SWMS or pre-start meeting)?

  • Rescue and retrieval equipment for the specific confined space activity must be available at the work site?
