Training

  • Have the entrants been trained in the hazards of the entry, the signs of exposure, to properly use the equipment and communicate with the attendant?

  • Have the attendants been trained in the hazards of the entry, to recognize behavioral effects of hazard exposure in authorized entrants and to call for rescue services if necessary?

Equipment and PPE

  • Was a confined space entry permit completed?

  • Date and time of the permit specified?

  • All persons identified with names and/or signatures where required? (entrant(s), attendant, entry supervisor and permit issuer)

  • Was the nature of the work specified?

  • Was air quality testing required, and if so was it documented with a signature?

  • Were the appropriate preventive measures checked off on the permit?

  • Has the entry supervisor signed off on the permit indicating that entry conditions are acceptable?

  • Was the completed entry permit posted near the confined space entrance for all involved to observe?

Attendant

  • Was monitoring done throughout the work period?

  • Did the attendant remain in visual, auditory or other type of communication with the entrant?

  • Does the attendant know the steps to follow in the event of an emergency?
