JOB SETUP

  • Is the OSHA poster displayed?

  • Has a clinic been located for the job and is the location posted?

  • Is there a porto-let located on the job?

HOUSEKEEPING

  • Does the job appear to be neat and organized?

  • Is trash and debris being contained into proper trash receptacles?

PPE

  • Are all employees wearing company issued hard hats?

  • Are all employees wearing eye protection?

  • Is the eye protection adequate for the task they are performing?

  • Are all employees wearing hi-viz clothing?

  • Are all employees wearing the proper work attire; T-shirts w/o arms cut out, pants w/o frayed holes?

  • Are ear plugs available on the job?

  • Are they being worn when required?

  • Are dust masks available on the job?

  • If a chain saw is in use on the job, are chain saw chaps available?

ENVIRONMENTAL

  • Is there a spill kit located on the job?

  • Is the spill kit accessible?

  • Is all equipment on the job free of fluid leaks?

  • Is there an absorbent boom spanning the creek/stream on each side of the job?

  • Is the erosion control barrier properly installed around the perimeter of the job?

Pickup trucks & equipment

  • Is the pickup cab and exterior relatively clean?

  • Is the bed of the pickup free of unnecessary trash and job site debris?

  • Is the cab of the equipment being maintained and in clean condition?

  • Are all company vehicles and equipment, both rented an owned free from damage?

Tools

  • Are all tools in good working condition, i.e. cords free from damage, auxiliary handles being used, etc.?

  • Are all hand held power tools plugged into a GFCI?

  • Are tools being properly stored?

  • Are hand tools being used for their intended purpose?

  • Are wooden hammer handles free of cracks or damage which could cause the handle to come loose from the head or cause the handle to break?

  • Are the auxiliary handles on tools where required?

  • Are the required guards on all hand tools in place?

  • Are all chain falls in use in good working order?

  • Are all cable come-a-longs in use in good working order?

Fall Protection

  • Is there fall exposures greater than 6 feet on the job?

  • If so, is fall protection being worn by those employees?

  • Are all employees tied to a sufficient anchorage point?

  • Was fall protection inspected by the user and is it free of defects?

Ladders

  • Are ladders being used on the job?

  • Are all ladders on the job in good repair?

  • Are the ladders being properly used; i.e step ladders opened with the spreaders locked?

  • Are extension ladders placed at the proper angle with the feet on firm level ground?

  • If used for accessing a work area, does the ladder extend 3 ft. above it's resting point?

  • Are the ladders tied off to prevent tipping?

  • Are there means to hoist hand tools up the ladder so employees can maintain 3 points of contact?

GENERAL

  • Is there a fire extinguisher on the job?

  • Are the fire extinguisher(s) accessible?

  • Are all gas and diesel cans on the job marked accordingly and in good condition?

  • Do all cans have a spark arrestor inside the spout?

  • Is traffic control being used on the job?

  • If NO, move on to next category. If YES, are the signs being properly maintained?

  • Are the signs displaying the proper warning?

  • Are the signs placed an adequate distance from the workers?

  • Are pneumatic tools in use on the job? If yes, are all hose connections secured together to prevent disconnect?

  • Are tool box talks being conducted on a regular basis with the crew?

Cutting and Welding

  • Have all flammables/combustibles been removed from the immediate area?

  • Are compressed gas cylinders stored correctly? Separated and secured upright?

  • Are all cylinders in use secured into a cart?

  • Is there a fire extinguisher available for the hot work operation?

  • Are all welding leads and hoses in good condition? Free of damage or leaks?

  • Do all torch setups have a spark arrestor at the torch and/or at the gauges?

  • Are the gauges in good condition and working properly?

  • Are all cylinders which are not in use properly capped?

Rigging

  • Was all rigging inspected by a competent person prior to use?

  • Are all slings/chokers in use in good condition and free of damage which may reduce the capacity?

  • Has the rigging in use been chosen and determined to be of sufficient capacity by a qualified rigger?

  • Has the weight of the item being picked been provided?

  • If weight is unknown, was an estimate calculated by a competent person?

  • Is the rigging being performed by a qualified rigger?

Cranes

  • Does the operator have the necessary certifications to operate the crane being used?

  • Was a daily inspection performed on the crane prior to use?

  • Is the operator being signaled by a certified signal person?

  • Is the crane setup properly; all outriggers properly deployed, all tires are off the ground, is the crane level?

  • Is the crane setup at least 20' from any overhead power lines?

  • If yes, have the proper steps been taken to maintain a safe distance from the power lines?

  • Is the use of a tag line necessary to control the load?

  • If yes, is there a tag line being used?

  • Has a "Pre-lift Checklist " been completed for the picks being made?
