  • Mechanical devices available to assist in moving materials (ex. pallet jack, forklift, chain fall, tugger, etc.)

  • Forklifts have been inspected and are free from damage or defects

  • Employees have been trained and are authorized to operate the equipment

  • Duro employees utilizing carts and/or tri-vices to hold and secure materials when cutting

Electrical

  • Extension cords in good condition, no missing ground prongs and damage

  • Temporary lighting adequate and cages/bulbs in place

  • Temporary power devices in good condition

  • LOTO in place and being used properly

  • Employees performing LOTO have been trained and are authorized to perform such work

  • Electrical panels secured and adequately protected

Fall Protection

  • Stairs are accessible and free of clutter or debris

  • Stair pans are filled and safe for use

  • Duro ladders are available for use and adequate for work being performed

  • Duro ladders have been inspected and are free from damage or defects

Heavy Equipment

  • Scissor Lift have been inspected and free from damage or defects

  • Duro employees have been trained and are authorized to use the scissor lift

Trenching/Excavations

  • Competent Person on site

  • Safety notified of excavation work

  • Soil classification determined and protective system provided (for trenches/excavations greater than 5' in depth)

  • Access/egress (ladder) provided for trenches/excavations greater than 4' in depth

  • Spoil piles and equipment set at least 2' back from edge of excavation

Cranes/Rigging

  • Crane Pick Plan completed and safety notified of pick

  • Duro employees trained and qualified for rigging

  • Duro employees trained and qualified for signaling crane

  • Review of crane pick discussed with crane company and crew performing work

  • Rigging equipment inspected and free from damage or defects
