  • Traffic Control Measures have been discussed and all necessary are in use, to include flashers, beacons, cones, traffic vests, signs, barricades and flaggers.

  • All hazards associated with the job have been discussed.(Add notes if necessary)

  • All work procedures involved have been discussed.(Add notes if necessary)

  • Energy source controls have been discussed to include: direction of feed, non-reclose operation, Lockout/Tagout, de-energize location and proper grounding of lines.

  • PPE requirements have been discussed and all PPE has been inspected and in good condition.

  • All special precautions and concerns have been addressed. Please list any in notes section.

  • Job site has been thoroughly surveyed looking for holes, poison ivy or any other hazards. WALK THE JOB SITE.

  • Provide pictures of job site and anything special that should be noted.
