The form must be filed as soon as practical, after addressing immediate safety concerns, or before end of shift, but never to exceed more than 24 hours following an incident. Upon Completion of this form, you must contact the DPA Kenneth Delaune at 504-376-7285. -
(UAC) Unsafe Act or Condition: hazardous condition or behavior which COULD directly permit the occurrence of an incident.
- (NM) Near Miss: A potential hazard or incident in which no property was damaged, and no personal injury was sustained, but where, given a slight shift in time or position, damage or injury easily could have occurred.
- (LOPC) Loss of Primary Containment: Unplanned/ uncontrolled release of material whether into the environment or secondary containment.
-Mechanical failure- Any instances where equipment onboard fails and causes a disruption to operations.
- Vessel Incident – unintended event that disturbs normal operations (allision, collision, grounding, loss of propulsion, black ship, etc.)
- Illness: Any abnormal condition or disorder resulting from a non-instantaneous event or exposure in the work environment.
- (FAC) First Aid Case: Medical attention that is usually administered immediately after the injury occurs and at the location where it occurred. If often consists of a one-time, short-term treatment and requires little technology or training to administer.
- (MTC) Medical Treatment Case: Any work-related incident where the involved parties received medical treatment other than first aid.
- Fatality- Employee death resulting from a work- related incident or exposure.
-Commercial Delay- Any event that creates a delay in commercial operations
