Underground utilities marked/identified before digging?

Digging around underground utilities performed with active UPC dig ticket?

Hand digging performed within 2' of marked lines, or hand digging performed until marked utilities identified before machine digging?

Excavations, adjacent areas and protective systems inspected by a "competent" person daily prior to the start of work and after each rain?

Excavations/trenches over 4' deep proper sloping applied (1-1/2' W to 1' V), or trench box used?

If trench boxes are used, is tabulated data on site?

Workers working within trench box at all times?

Ladders placed within 25' of workers?

Spoil banks at least 2' away from edges of cut?

Barriers installed protecting unattended and remote excavations/trenches?

Warning vests or highly visible clothing worn by workers exposed to traffic?

Workers not exposed to overhead suspended loads and maintaining safe distance from vehicles loading/unloading?

Audible alarms functioning properly on excavation equipment?

Workers wearing appropriate PPE? Ex. Hard hats and safety glasses.
