Pre Assessment

  • The training certification has been reviewed by the assessor prior to conducting test

  • The agency employee has been given a full induction prior to assessment with particular emphasis given to the forklift safe steps material

  • In the event that either of these pre-assessment activity is not complete the assessment must not proceed until both pre-assessment activities have been confirmed completed by the assessor,.

Conducting the Assessment

  • A commentary driving assessment is where the person under assessment gives a running commentary of what they are looking at, what they are doing and what they are thinking while driving the forklift.

    The assessment will be terminated if the person under assessment commits a safety breach as listed below.

    In the event of a termination the person under assessment should be advised to park the forklift and remove himself from the vehicle.

Safety Breach

  • The person under assessment is involved in an incident or near miss

    The assessor feels unsafe due to the actions or information of the person under assessment.

Miscellaneous

  • Depending on the situation questions can be asked before during and after the performance of the tasks or even a combination of the above. However you should use discretion in the timing of asking and recording answers to the questions especially when the person under assessment requires concentration to complete the task at hand.

    You can clarify a question to the person under assessment, but you must never provide assistance with the answer.

    In order to be deemed competent the person under assessment must demonstrate both knowledge and good practice in all areas of the test.

Assessment

  • The assessment should involve a range of basic forklift manoeuvres appropriate to the task that will be undertaken which require the person under assessment (referred to as the driver) to demonstrate a level of competence in the operation of the forklift and both spatial and safety awareness.

  • Does the driver take time to familiarise themselves with the vehicle and perform pre use checks?

  • Detail the failures exhibited by the driver

  • Does the driver put on the seat belt without having to be told?

  • What is the assessment of the drivers general confidence in the use of the vehicles controls?

  • Provide information to substantiate your scoring

  • What is the assessment of the drivers spatial awareness when manoeuvring loads?

  • Provide information to substantiate your scoring

  • What is the assessment of the drivers control of loads incl. position of fork arms in relation to the load, load stacking, load stability etc.

  • Provide information to substantiate your scoring

  • What is the assessment of the drivers style? (starting and stopping, speed appropriate to task, braking, general manoeuvring)

  • Provide information to substantiate your scoring

  • Does the driver fully turn head and maintain position in the direction of travel while reversing?

  • Provide information to substantiate your answer

  • What is the assessment of the drivers safety awareness during completion of the activities?
