GENERAL

  • Has the operator been instructed in the safe operation of this type of lift?

  • Ensure Annual Inspection has been completed within the last 13 months.

  • All gauges operate properly.

  • Hourmeter operates properly.

  • Presence of fire extinguishers

  • Overhead cage and seat condition?

  • Condition of forks?

  • Driving lights and indicators operate properly. (if applicable)

  • Check front, top, side and rear wear pads for presence of grease.

CAB AND ELECTRICAL

  • General appearance; no visible damage.

  • Frame level indicator and window glass undamaged and clean.

  • Gauges, switches, joysticks, foot controls and horn operational.

  • Seat belt in good condition and catches properly.

  • Frame level and boom indicator are working?

POWER UNIT

  • Engine oil, coolant, and battery fluid levels correct.

  • Transmission operates correctly, transmission fluid level correct.

  • Engine air filter clean and secure.

  • Engine R.P.M. correct.

  • Engine fuel level

  • Radiator, oil cooler clean.

  • Steering in both directions with engine at low idle

  • All joystick functions operate smoothly and correctly.

  • Fuel, oil filters secure and not leaking.

DRIVE TRAIN

  • Wheel/Tire Assembly—Properly inflated and secured; no loose or missing lug nuts. Inspect for worn tread, cuts, tears or other discrepancies.

  • Mirrors clean and undamaged

  • Service brake and parking brake operation

  • Battery cables tight, no visible damage or corrosion.

  • Forward and reverse travel.

  • Engine access doors closed and properly secured.

  • Horn and Backup alarm operates properly.

HYDRAULIC SYSTEM

  • Hydraulic oil level is correct.

  • Pivot pins secure; hydraulic hoses undamaged, not leaking.

  • Hydraulic controls go up, down, extend, retract?

  • Front Axle - Steer cylinders undamaged, not leaking; pivot pins secure; hydraulic hoses undamaged, not leaking.

  • Outriggers - Pins secure; hydraulic hoses and cylinder undamaged, not leaking.

  • Rear Axle—Steer cylinders undamaged, not leaking; pivot pins secure; hydraulic hoses undamaged, not leaking.

SAFETY AND OPERATOR’S MANUALS, DECALS

  • Manuals & Required Documents. In storage box, in good condition & legible.

  • Labels. In place, secure & legible.

  • Load Chart is correct and legible?

  • Controls, pedals and indicators are working correctly?
