  • This Forklift Inspection is to be completed by a competent person with the relevant High Risk Work licence for the forklift being inspected (i.e. LF\LO).

    The checklist is to be completed at the commencement of each work week or before the forklift is used for the first time following any maintenance or repairs.

    The relevant area management/supervision is to review all completed Forklift Inspections for their area monthly to ensure they are being completed as required and that all items noted are action as required.

Tyres, Attachments and Body

  • All obstacles have been removed from around forklift e.g. other vehicles, plant, equipment witches hats etc?

  • Forklift is free of leaks/visible debris under the body or on the ground under the forklift?

  • All tyres are in good condition i.e. no uneven wear, cracks, gouges or penetrating objects (e. g. nails)?

  • Tyre pressure is within the specified range?

  • Tyre rims are in good condition e.g. no dents, deformation of rims or damage to stud holes?

  • All attachments are securely connected and functioning as required?

  • Apron and overhead guard are secure?

  • Body has been inspect and there are no new bends or dents that have already been noted?

Fitting - Seats, Lights, Hydraulics and Lifting Gear etc.

  • Seat-belts are working (where fitted)?

  • Seat belt and steering wheel are secure and adjusted as required?

  • Seat is in reasonable condition?

  • Lifting slings/chains and guide are fit for purpose with no damage or unacceptable wear?

  • Hydraulic Cylinders are leak free and pistons are clean and free of corrosion and/or wear?

  • All lights and beacons are working e.g. road lights if fitted, flashing beacon?

  • Reversing alarm and steering wheel horn operational?

Battery, Engine and Fuel

  • Battery is undamaged and if not sealed unit acid water level within required range?

  • Battery is charged i.e. able to start forklift and charging i.e. shows charge on gauge if fitted on dash?

  • Engine oil is at a reasonably level between markers on dip stick?

  • Brake and radiator fluids are at reasonably level between markers on respective containers?

  • Engine and engine bay are clean and all belts appears to be in a serviceable conditions with no apparent damage to any item.

  • Fuel levels are suffice for work period and refeuling is available on site e.g. pump station or additional gas cylinders?

  • If gas cylinders fitted, they are not leaking and are correctly secured in the craddel provided?

  • Other Items To Report

Controls, Pre-start, Fluid, Oils and Hydraulics

  • Hydraulic fluid is at a reasonably level between markers on sight-glass?

  • All visible hydraulic hoses and fittings are in a serviceable condition e.g. not perished or damaged or leaking?

  • All attachments and lifting equipment are fitted with compliance plates (if required) and all relevant inspections are current?

  • Forklift's controls including pedals are clearly marked and in servicable condition and operate on startup of forklift?

  • During start up and pre-operation checks all hydraulic cylinders seat and extend correctly and there are no leaks ?

  • Engine cooling system warmed up to normal level and there are no coolant leaks?

Brakes

  • Hand Brake and Foot Brake are operational and working as required?

  • Load capacity plate, if fitted, is legible and correct?

  • Brakes and hand controls are working together as required?

  • Safety Pedal is working as required?

Steering

  • Steering Wheel and Knob are free of damage?

  • Steering is smooth from lock to lock?

Corrective Actions

  • Forklift has been tagged out by me and remains out of service?

  • Arrangements have been made to have all issues corrected e.g. forklift serviced, repaired etc?

  • Incident Report been raised for all potential or actual Class 1 and Class 2 Incidents/Issues?

  • All issues have been corrected and I am returning this forklift into service?
