I. PRE-OPERATIONAL INSPECTION

  • Review the Forklift safety check list.

  • Review Operator's controls.

  • Review Refueling/Recharging procedures.

II. BASIC OPERATION

  • Operator checks for overhead obstructions.

  • Forklift is centered to the load.

  • Operator stops before raising/lowering forks.

  • Forks fully penetrate into the load.

  • Tilt/Lift the load

  • Is the load stable on the forks.

  • Operator looks behind when backing out slowly.

IV. TRAVELING WITH A LOAD

  • Operator looks in the direction of travel.

  • Operator travels at a resonable speed.

  • Operator sounds the horn at Intersections/Doorways/Corners

  • Operator looks behind before backing.

  • Operator traveling up Ramps/Incline.

  • Breaking with a load.

  • Lift/Lower load while traveling.

  • Hight of forks from floor (2-4 inches)

  • Double loads/awkward loads.

V. STACKING LOADS

  • Forward movement

  • Position of load on Stack/Rack.

  • Operator looks behind before backing.

  • Forks clears the load before Lifting/Lowering.

VI. TRUCK LOADING/UNLOADING

  • Truck/Trailer is chalked.

  • Breaks on truck/trailer are set.

  • Dock boards/Bridge plates are in place.

  • Travel up ramp in forward direction with load.

  • Travel down ramp in backwards direction with load.

  • I certify that the information is correct and accurate . Evaluator/Trainers signature.
