Safety Inspection/Audit

Housekeeping

  • Is debris secured and/or removed from floors?

  • Is CAZ properly marked out?

  • Is CAZ monitor in place?

Excavations/Trenches (use N/A if there are no excavations/trenches)

  • Are utility mark outs in place?

  • Are adjacent buildings undermined or in danger of being undermined?

  • Was the daily inspection by the competent person performed?

  • Are materials/equipment located away from unprotected trench?

  • Is there shoring installed if excavation/trench is deeper than 5 feet?

  • Is the shoring adequate?

  • Is there adequate egress from excavation/trench?

  • Is there fall protection (required at > 6 feet)?

Electrical Safety

  • Are GFCI’s in use?

  • Are all extension cords free of defects?

  • Are all tool cords free of defects?

  • Are ground pins on cords in place and not missing?

  • Are electrical panels and boxes closed or otherwise protected?

  • Are LO/TO procedures being used correctly?

  • Are electrical lines and facilities being protected from contact?

Construction Equipment

  • Is the swing radius barricaded?

  • Are the backup warning devices working?

  • Is there roll over protection?

  • Is there a fire extinguisher on vehicles?

  • Was the daily inspection performed?

  • Is the equipment parked/safeguarded when not in use?

Ladders

  • Are all ladders in use inspected daily?

  • Are ladders in good shape and free of defects?

  • Are ladder permits being issued (for new DB projects)?

  • Are ladders properly set up on level surface?

  • Are A-frame ladders being used fully open?

  • Are extension ladders not separated?

  • Are ladders footed or cleated at bottom?

  • Are ladders secured from movement at top?

  • Do ladders extend 3 feet past the landing area?

Confined Space

  • Are confined spaces properly marked?

  • Is the confined space entry program in place?

  • Is a confined space permit posted at entry point?

  • Is the attendant on duty at entry point?

  • Is a calibrated gas meter available and in use at entry point?

  • Is the rescue tripod properly set up?

  • Are there any hazards not identified on the confined space permit?

PPE

  • Are all workers wearing hard hats?

  • Are all workers wearing reflective vests?

  • Are all workers wearing safety glasses?

  • Are all workers wearing work boots with safety toe?

  • Are workers using or near high noise tools wearing hearing protection?

  • Are workers who are using grinders, chipping hammers or other tools creating flying debris wearing face shields?

Street Operations/MPT

  • Is MPT properly set up per drawings/permit?

  • Are DOT permits up to date and available?

  • Are certified flaggers being used as needed to move equipment?

  • Are concrete jersey barricades in use against traffic (for new DB projects)?

  • Is MPT set up to keep pedestrians out work area?

  • Are proper traffic warning and directional signs set up?

  • Are there signs to direct pedestrians?

  • Is the entry to MPT area closed when not moving equipment/material in or out?

  • Is a 5-foot-wide pedestrian walkway maintained?

  • Are working warning devices mounted on traffic side of street MPT?

  • Are MPT barriers, barricades, fences and netting in good working order?
