Maintenance in the field is consider high risk as support tools and equipment such as lifting gear is not available. “Man Machine” interaction risk is heightened and must be identified and managed. Strict supervision must be taken by crew Manager for any maintenance issues. Only nominated competent workers are to undertake day to day maintenance work

Step 1- Identify the machine fault if possible

Advise management and follow any instructions given by Foreman (management)

Step 2- If possible and management has agreed, move machine to a safe area away from any other activity. Make sure there is enough space around the machine to allow smaller service trucks to access if required

If a machine is unable to be moved as result of a breakdown, consideration must be given to “tracking” into the machine. Tracking must be wide enough (4meteres min) to allow safe travel of any service vehicle, (4x4 Utes). Consideration must be given to creating a 360 degrees access around the machine requiring maintenance as another machine may be required to assist any lifting phase. This must be considered “high risk” as it will create a “man machine interaction” risk that must be managed

Step 3- Set up plant so it is safe for repair work, ensure all energy source, electrical, hydraulic and gravity is shut off and isolated, lower any hydraulic attachments . At no time should any person be working under a raised object.

Follow any manufacture recommendation for setting up specific plant

Isolate any attachment from its power source

Step 4 – Test to ensure isolation process is effectively working and remove key

Step 5- Attach tag, (Lock out tag out tag) ensure the tag signed and dated. (NB internal training on the purpose of tags and who can remove them will be required, see lock out tag out procedure)

Step 6 – Handover, if an external provider or speciaist is used to repair plant, discuss with them what has been done to this point. Suggest that they recheck isolation steps 2, 4, 5 before undertaking the repair work

Prior to any start up all guards must be put back in place, this is a must do!
