  • Were atmospheric results recorded on H&M confine space entry form?

EXCAVATION H&M Safety Handbook - Section 21.1

  • Is the excavation checklist complete prior to all excavating activity?

  • Cutting Woods (timbers, etc.): Is the employee wearing a face shield, chaps and ear plugs while using the chain saw?

  • Are permanent identification tags attached to the rigging equipment? (22.4.2.4)

  • Was all rigging equipment inspected and is in good condition - (non suspect or counterfeit chains are to be used?) (22.2.2 / 22.2.3)

  • Was the weight of the load determined and was the proper size for slings and components made-out? (22.3.1.1/22.3.1.2)

EQUIPMENT

  • Is the condition of the equipment in good shape?

  • Was equipment inspected before use?

  • Does the tool cord and/or extension cord have a current monthly inspection sticker? (in accordance with H&M assured grounding program)

  • Are employees using the right equipment for the job?

Hot Work (PGL A4.4.1 Pg. 77)

  • Is a fire watch present for at least 30 minutes to ensure no flare up?

  • Are cylinders chained and stored properly in an upright position with the valves closed? Are cylinders stored away from oil, grease, chemicals and other combustibles?

  • If hot work is taking place, is the hot permit complete in its entirety and is at least 50ft. away from where work is taking place?
