Donation Center Training and 30, 60, 80, Day Review Audit

  • Use this checklist to evaluate a new employee's performance (new hire OR transfer into the department) by completing at 30 days, 60 days, and 80 days after employee joined the department/location.

    Please submit the 30-60-80 day review to the Director of Employee Relations for all "1" ratings.

    Any employee who receive a "1" ratings on their 30-60-80 day review may receive further disciplinary action, up to and including termination, for poor performance.
