    3. If you select At Risk for any item, the LADDER must be Removed from Service. Add photos and notes by clicking on the paperclip icon.

Section 1.0 - Ladders (Stool, Step, Portable- Extension, Fixed, and Rolling)

  • 1.1 Identify the Construction of Ladder.

  • 1.2 Identify the area of inspection.

  • 1.3 Are the warning and duty rating labels easy to read?

  • 1.4 Does the ladder require service, maintenance, or removal from service?

  • 1.5 Is the ladder free from oil, grease or other material?

  • 1.6 Does the ladder show signs of rust?

  • 1.7 Are the rungs, cleats or steps intact and free from damage?

  • 1.8 Are there any missing parts from the ladder such as a rung, fly wheel, or rail?

  • 1.9 Do the rails have cracks or splintering?

  • 1.10 Is the ladder warped, bent, uneven, or damaged?

  • 1.11 Has the ladder been hit?

  • 1.12 Is the ladder free from protruding objects that could cause injury?

  • 1.13 Has a repair been made to the ladder other than those allowed by the manufacturer?

  • 1.14 Do rolling step ladders have a locking device to prevent movement?

  • 1.15 Do the spreader and hinges bars lock when opening the step ladder?

  • 1.16 Have impacted employees been trained on ladder use?

  • 1.17 Fixed Ladder - Are fixed ladders (24 feet or taller) equipped with a fall restrain system?

  • 1.18 Fixed Ladder - Are guard rails and chains attached or closed at the top of the ladder landing?

  • 1.19 Extension Ladder - Is the rope and pulley available on the ladder?

  • 1.20 Extension Ladders - Are the rung locks available and working without defect.

  • 1.21 Extension Ladder - Are the rails racked, bent, split, or warped?

  • 1.22 Rolling Step Ladder - Is bracing (vertical and horizontal) free of bends, loose bolts and nuts, and damage?

  • 1.23 Rolling Step Ladder - Are caster wheels free of damage?

  • 1.24 Did you observe an Un-safe condition? (Example - Broken or Damaged Equipment)

  • 1.25 Was the Un-Safe Condition corrected?

  • 1.26 Did you observe an Un-Safe Act or Behavior? (Example - Shortcut)

  • 1.27 Was the Un-Safe Act or Behavior Corrected?
