General Site Requirements

  • Do employees have safe access to the site?

  • Are required OSHA, Right-to-Know, and other Department of Labor posters displayed adequately on the job?

  • Are emergency phone numbers posted?

  • Are SDSs and Site Safety Plans available to all employees?

  • Do employees have adequate access to drinking water, toilets, eyewash, emergency shower, and other sanitary facilities?

General Site Safety Conditions

  • Are employees protected from impalement hazards?

  • Is lighting (temporary, permanent, or natural) adequate (generally 5 foot-candles for most construction areas)?

  • Are walkways clear, especially emergency egress?

  • Are general-use fire extinguishers available (one for every 3000 sqft) and inspected regularly?

Personal Protective Equipment

  • Do all employees have the minimum PPE required by company policy (hardhat, safety glasses, shirt with minimum 4" sleeves, adequate work pants, gloves, and work boots)?

  • Are employees using required task-specific PPE?

  • Is PPE in good shape and has it been inspected before use?

Temporary Power

  • Are GFCIs in use and tested daily?

  • Are temporary cords in good condition?

  • Do temporary lights have proper guards and supports?

  • Do temporary panels have adequate covers?

  • Are electrical dangers posted?

Hand and Power Tools

  • Are hand and power tools in good shape and inspected before use?

  • Is the correct PPE available and in use for power tools being used?

  • Is a defective equipment tagging procedure in effect?

Ladders

  • Are ladders inspected before use and in good condition?

  • Are ladders sized adequately for work being performed?

  • Are ladders being used properly (secured, step-ladders fully opened, step ladders extending 3' past upper landing)?

  • Are defective ladders removed from service in a way that prevents others from using them?

Scaffolding

  • Is there any scaffold in use on site?

  • Has the scaffold been built under the supervision of a qualified person?

  • Has the scaffold been inspected before use for this shift?

  • Are guardrails and toeboards installed on all open sides per OSHA requirements?

  • Is there adequate clearance between the scaffold and live electrical lines?

  • Are employees using the scaffold trained on the hazards associated with its use?

Motor Vehicles

  • Does each truck have a fire extinguisher?

  • Does each truck have a first aid kit?

  • Are company vehicles (owned or rented) being operated by only qualified operators?

Welding and Cutting

  • Is there any welding or gas-cutting activity on site?

  • Is proper PPE being used (adequately shaded eye/face protection, proper gloves, sleeves/jacket)?

  • Is equipment in good operating condition and grounded?

  • Are in-use cylinders secured to prevent tipping?

  • Are gauges removed and caps replaced when cylinders are not in use?

  • Are cylinders stored properly (minimum 20' apart or separated by 5' high, 1/2 hour rated partition, secured to prevent tipping)?

Fire Prevention

  • Are workers and materials protected (either by distance or shielding) from hot work?

  • Are adequate fire extinguishers on hand (and inspected) for hot work?

  • Are site-required hot work permits in place for all hot work?

  • Is a fire-watch designated, trained, and at the scene of hot work?

Confined Spaces

  • Are there any known confined spaces on site?

  • Are confined spaces located and marked?

  • Does Mainelli Mechanical have any work in the confined spaces?

  • Is all required equipment on site (air monitor, ventilator, rescue system)?

  • Is the competent person on site?

  • Has an hazard analysis been performed of the space?

  • Have proper permits, declassification, or other required paperwork been filed as required?

  • Have all workers been trained on confined spaces (proof of training required on site)?

Fall Protection

  • Are all floor and wall openings adequately guarded?

  • Are fall protection systems in place to protect employees working on roofs?

  • Are there any fall hazards on site that require the use of PFAS?

  • Is a competent person on site?

  • Are all workers trained on equipment use?

  • Has a fall protection plan been developed including rescue?

  • Has all equipment been inspected, including periodic inspection by a competent person?

  • Is all equipment being used properly?

Excavations

  • Are workers in and around excavations trained in hazard recognition?

  • Are there any trenches deeper than 4 feet?

  • Is a competent person on site?

  • Has the trench been inspected before work begins, and after events that effect its stability?

  • Is the written Daily Trench Inspection sheet on hand for all trenches?

  • Has a soil classification test been performed?

  • Is the trench being monitored for hazardous atmosphere?

  • Do workers have adequate access/egress (ladder or slope within 25' of travel)?

  • Is traffic adequately barricaded to keep it away from trenching operations?

  • Are workers exposed to traffic (vehicle or equipment) wearing hi-vis outer layer?

  • Are there any trenches deeper than 5 feet?

  • Are adequate protective systems in place (sloping, benching, shoring, or shielding)?

  • Are protective systems being used properly?

Subcontractors

  • Are subcontractors on site aware of safety requirements?

  • Any other safety or liability concerns?
