When assessing the level of risk - use the following Risk Rating Matrix and follow the instructions related to your estimated level of risk.

1.0 Access to Task Area

  • 1.1 Is there the risk of Slips, Trips or Falls on the route to the Task Area?

  • If the answer is "Yes" - Evaluate the level of risk using the slider

2.0 Work at Height

  • 2.1 Is there safe access to the Task Area?

  • If the answer is "No" - Evaluate the level of risk using the slider

  • Are "Reasonably Practicable" controls available to be introduced? If "Yes" - List the controls

  • 2.2 Is there adequate edge protection around the task area?

  • If the answer is "No" - Evaluate the level of risk using the slider

3.0 Work Equipment

  • 3.1 Is there a risk of being injured whilst using hand tools or electrically operated tools and equipment?

  • If the answer is "Yes" - Evaluate the level of risk using the slider

  • Are "Reasonably Practicable" controls available to be introduced? If "Yes" - List the controls

4.0 Manual Handling

  • 4.1 Is there a risk of Musculoskeletal Injuries from Manual Handling Activities?

  • If the answer is "Yes" - Evaluate the level of risk using the slider

  • Are "Reasonably Practicable" controls available to be introduced? If "Yes" - List the controls

5.0 Hazardous Substances (Liquids, Dusts, Mists etc)

  • 5.1 Is there a risk of your health being adversely affected whilst using hazardous substances or carrying out work activities that produce hazardous substances?

  • If the answer is "Yes" - Evaluate the level of risk using the slider

  • Are "Reasonably Practicable" controls available to be introduced? If "Yes" - List the controls

6.0 Noise

  • 6.1 Is there the risk of your hearing being damaged by high noise levels?

  • If the answer is "Yes" - Evaluate the level of risk using the slider

  • Are "Reasonably Practicable" controls available to be introduced? If "Yes" - List the controls

7.0 Vibration

  • 7.1 Is there a risk of Hand Arm Vibration Syndrome (HAVS) as a result of increased vibration levels?

  • If the answer is "Yes" - Evaluate the level of risk using the slider

  • Are "Reasonably Practicable" controls available to be introduced? If "Yes" - List the controls

I confirm that a "Suitable and Sufficient Assessment" of the latest hazards has been carried out and the Method Statement/ Method of Working has been developed to include all the necessary control measures.
