Equipment Worthiness

  • Is the equipment clean? (Ex: interior and exterior)

  • Is the truck free from any noticeable hose leaks? (Ex: oil, hydraulic, coolant)

  • Does the hydraulic hose have all the proper clamps and zip ties in the proper places?

  • Has the operator checked the engine oil?

  • Did the operator check the coolant level at the beginning of the day?

  • Is the front engine belt in good condition?(Ex:no wear or tears)

  • Is the hydraulic hose free of any noticeable spots do to rubbing?

  • Is the Hydraulic oil sight glass full?

  • Is the hose reel in good condition? (Ex: no peeling of the hose, couplings are in good shape, using leader hose)

  • Is the debris body in good condition? (Ex: rot spots, pin holes)

  • Is the cyclone in good condition? (Ex:holes, trap door is clean)

  • Are the water pump filter housing and filter in good condition? (Ex: holes, filter is clean)

  • Is there enough oil on the water pump dipstick?(supervac, clean earth, guzzler only)

  • Is the blower air filter clean and in good condition?(Supervac and Vactor only)

  • Is the blower oil sight glass full?

  • Do all of the pressure gauges work properly?

  • Is the truck free of problems with any of the following: air gauges, warning lights, low air, temperature gauge and voltage gauge.

  • Is the heating system (defroster) in working order?

  • Is the radiator hose in good condition?

  • Is the boom of the truck sitting on its latch (not up in the air) and does the cab alarm work when the body is up?

  • Are all lights working properly? (running, brake, backup, backup alarm)

  • Has the operator greased the chicksand valve, reel bearing, rotate bearing, table bearing, rear door bearing, and debris body door bearing?

  • Is the vacuum jumper hose in good condition?

Jobsite Setup

  • Does the driver have a plan for the day? Has he reviewed maps prior so he has his day set up?

  • Did the operator properly set up the job site? Are the following being used? Road hose, ramps, flaggers, wheel chocks, cones and manhole cage

  • Is the driver aware of any overhead obstructions on the site?

  • Did the driver engage the work mode properly as per the instructions?

  • Does the operator know proper procedure if something goes wrong with the truck? Both while driving or during operation.

  • Is the truck equipped with absorbent pads/speedi-dry?

Productivity/Safety

  • Does the operator have proper equipment and permit forms for confined space entry?

  • Are operators wearing all there PPE? (Ex: safety vest, hard hat, safety glasses, steel toe boots, gloves, hearing protection)

  • Is the truck equipped with the necessary tools to do the job? ie.hook, nozzle, tubes etc.

  • Does the operator have the proper flash light?

  • Did the operator check the transfer cases (engaging the truck to see if it's operating the proper way) so the truck doesn't take off on driver while working.

  • Does the truck have proper safety equipment? (Ex: fire extinguisher, three triangles, med kit, wheel chocks, Spill Kit)

  • Does the driver have spare fluids on the truck and are they properly labeled?

  • Trucks nozzles have all inserts and does it have the proper inserts for that nozzle?

  • Is the the trucks water pressure gauge consistently showing between 1800 psi and 2000 psi while cleaning?

Road Worthiness

  • Is the Drivers Alert decal sticker affixed to the back of the truck, free of obstruction, and in good shape?

  • Is all the paperwork up to date? ( Ex: registration, insurance card, IFTA, HUT)

  • Does the driver have his driver's license and medical card?

  • Does the driver have his current DVIR and previous day's DVIR?

  • Is the driver's log book present and up to date?

  • Are the tires and rims in good condition? (Ex: cracks, treads, bolts tight)

  • Is the load secured? (Ex: tubes, hoses)

  • Are the right side and backup cameras operational?

  • Were all fluids checked and topped off if needed?

  • Are the mirrors in working order and are clean.
