  • 1. Is use of gasoline, LPG, or diesel powered forklift truck inside the warehouse prohibited due to the carbon monoxide hazard?

  • 2. Are personnel adequately trained and evaluated before they use MHEs/forklift trucks?

  • Are they re-evaluated at three year intervals?

  • 3. Are forklift operators pre-qualified to use forklifts prior to operation and does the supervisor ensure that only qualified personnel operate the MHEs/forklifts?

  • 4. Are all forklift trucks equipped with seat belts and used at all times when trucks are operating?

  • 6. Do the MHE/forklift operators ensure that the load is properly placed on the forks and that the MHE's stability is maintained?

  • 9. Do personnel conduct forklift truck safety inspections on a daily basis or every ten hours of use?

  • 11. Are drip pans provided in forklift parking areas to limit oil and lubricant contaminated floors?

  • 12. Do all MHEs/forklift trucks have a warning horn, which can be clearly heard above the normal noise in the areas where operated?

  • 13. Are the brakes on each forklift truck capable of bringing the vehicle to a complete and safe stop when fully loaded or, for MHEs, is power to the drive motor shut off when the operator releases his grip on the device that controls the travel?

  • 14. Are unattended, parked forklift trucks left with the parking brake set, key removed and forks lowered?

  • 15. Are fork-mounted work platforms used to lift employees equipped with 42" high railings and a screen or solid barrier on the side facing the forklift mast?

  • 16. Are fork-mounted work platforms used to lift employees secured to the forklift mast?

  • 17. Are employees provided with a Personal Fall Arrest System when on a fork-mounted work platforms and trained in its correct usage?

  • 18. Are the load capacity label, operation and maintenance instruction plates, tags or decals in place on the forklift and legible?

  • Battery Charging Operations

  • 1. Is the battery charging area properly vented to ensure that there is no dangerous buildup of hydrogen gas from charging batteries?

  • 2. Is the necessary PPE (rubber gloves, face shield, rubber apron) for battery maintenance and spill response available near the charging location?

  • 3. Is there an eyewash station near the battery charging area?

  • 4. Do warehouse personnel know how to clean up battery acid spills?

  • 5. Is a clean up kit with a supply of acid neutralizer located near the battery charging area?

  • 6. Are nearby wiring and electrical devices properly installed to ensure they do not overheat or produce an ignition source?

  • 7. Are there other ignition sources nearby and is their use controlled?

  • 8. Are "No Smoking" signs posted and does the supervisor enforce the requirement?

  • Ladders

  • 1. Are all ladders used by employees at their workplace in overall good condition?

  • 2. Are ladders free of sharp edges, splinters, splits, decay, or broken rungs, spreaders, locking clamps and side rails?

  • 3. Do ladders have no-slip feet?

  • 4. Are wooden ladders unpainted to prevent hiding the damage/cracks in the wood?

  • 5. Are the joints between the steps and side rails tight, all hardware, and fittings securely attached, and do the movable parts operate freely without binding or undue movement?

  • 6. Do stepladder spreaders or locking devices securely hold the front and back sections in the open position?

  • 7. Are ropes for extension leaders in good condition (not frayed or badly worn)?

  • 8. Are the rungs or steps clean (free of mud, grease, or oil)?

  • 9. Are the steps uniformly spaced, 1 ft. (0.3 m), and parallel and level when the ladder is in position for use?

  • 10. If there are any damaged ladders on the sites are they tagged for destruction or repair and kept out of service?

  • 11. Is the use of AC-powered tools or working on or near an electrical system or overhead power lines limited to wood or fiberglass ladders only (no metal ladders)?

  • 12. Are ladders OSHA/ANSI-rated Type 1 (250 lbs. or 113 kg), 1A (300 lbs. or 136 kg), or 1AA (375 lbs. or 170 kg) or equivalent?

  • 13. Are stepladders limited to no more than 20 ft. (6 m) long?

  • 14. Are straight ladder limited to no more than 30 ft. (9 m) long?

  • 15. Is the straight or extension ladder placed at a pitch of 1:4 (i.e., 1 foot or 1 meter horizontal for each 4 feet or 4 meter vertical)?

  • 16. Is the straight or extension ladder placed to prevent slipping or is the base and top tied in position?

  • 17. For any ladder placed in front of a doorway, is the door locked or blocked from the other side?

  • 18. Are ladder tall enough to reach the object required?

  • 19. Are ladders not placed on boxes, barrels, tables or other materials to gain additional height?

  • 20. Are workers staying at least lower than two steps from the top on a step ladder or four steps from the top on a single section or extension ladder?

  • 21. Are workers not climbing on the bracing on the back legs of the step ladder?

  • 22. Is there no more than one person on the ladder at a time?

  • 23. Are rolling stairs available and used instead of a ladder?

  • 24. Is a ladder is used to provide access to an elevated form, do the side rails extend at least 36" (91 cm) above the platform edge and secured?

  • Chemical Safety

  • 1. Are solvent-based glues, wood finishes, and thinners and other hazardous chemicals kept in appropriate containers that are properly labeled?

  • 2. Are flammable liquids properly stored in approved flammable liquid storage cabinets when not in use?

  • 3. When flammable or toxic chemicals are in use, is the ventilation adequate to ensure that vapor concentrations do not buildup in the shop?

  • 4. Are compressed gas cylinders stored in an outside roofed cage or otherwise secured against tipping over?

  • 5. Are "No Smoking - No Ignition Sources" signs posted where flammable materials are stored or used (indoors or outdoors)?

  • 6. Ware comprised gases such as oxygen separated from flammable compressed gases by at least 20 ft (6 m) or a half-hour rated firewall?

  • Electrical Safety

  • 1. Are electrical machinery and appliances grounded?

  • 2. Do all extension cords have a grounding conductor?

  • 3. Do plug adapters have the necessary ground conductor?

  • 4. Are all disconnecting switches and circuit breakers labeled to indicate their use or equipment served?

  • 5. Do all interior wiring systems include provisions for grounding metal parts of electrical raceways, equipment, and enclosures?

  • 6. Are all electrical conduits, raceways and enclosures securely fastened in place?

  • 7. Are all energized parts of electrical circuits and equipment guarded against accidental contact by proper cabinets or enclosures?

  • 8. Is sufficient access and working space provided and maintained around all electrical panels (36 ins.) and equipment to permit adequate access for maintenance?

  • 9. Are all unused openings (including knockouts) in electrical enclosures and fittings closed with appropriate covers, plugs or plates?

  • 10. Are electrical enclosures such as switches, receptacles, and junction boxes, provided with tight fitting covers, plugs or plates?

  • 11. Are protective covers installed on all light bulbs that could be impacted by personnel or inventory?

  • 12. Are electrical receptacles in high hazard areas (wet areas such as bathrooms, and exterior areas) provided with GFCIs?

  • Housekeeping

  • 1. Are walking surfaces kept dry or appropriate means taken to assure the surfaces are slip-resistant?

  • 2. Are walking surfaces free of debris?

  • 3. Are all spilled hazardous materials or liquids, cleaned up immediately and according to proper procedures?

  • 4. Is scrap, debris, and waste stores safety and removed from the worksite promptly?

  • Warehouse Vehicle Safety

  • 1. Are there enough seat belts for ALL workers riding in the vehicle?

  • 2. Are workers prohibited from riding in the back of a vehicle with the inventory?

  • 3. Is inventory properly secured inside the truck?

  • 4. Does the truck have a hydraulic lift gate?

  • 5. Are vehicles properly inspected and maintained?

  • 6. Are truck tires chocked when in the parked position for both off- and on-loading?

  • 7. Are signs posted to remind commercial trucks to chock their tires?

  • 8. Is the dock surface well marked and are the bumper pads installed to prevent damage to the truck and dock?
