CONTROLS - Operator to shut crane down before commencing to refuel. An earthing attachment should be used when refueling to prevent discharge of static electricity build up during operation. No smoking or naked flames during refueling.
-
Can anyone be using or near the plant SLIP, TRIP or FALL?

Uneven surface
Weather conditions
Slippery surfaces
CONTROLS - Keep hand holds, steps and floor clean and clear of oil, grease, mud, rubbish, and tools. Always use proper steps and hand holds when climbing onto or alighting from machine. Never exit or enter the crane cab by any other means than the access system provided (i.e., grab handles)
-
Are there ERGONOMIC - MANUAL HANDLING hazards associated with the plant?

Poor posture
Repetitive or sustained movements
Awkward positions
Strained movements
Poorly designed seating
Access and egress
Access for maintenance
Routine inspections and adjustments
CONTROLS - Where possible use mechanical devices to eliminate or reduce manual handling. If unable to use a mechanical device, ask for assistance. When placing heavy chains or slings onto hooks, lower / manoeuvre the hook as close as possible / safely to enable the minimal manual lifting as possible, also ask for assistance. Identified manual handling hazards are to be included in the SWMS for the use of the plant. Note that chains, slings, and blocks are extremely heavy and should be handled by two people. Appropriate Seat to be fitted.
-
Are there ERGONOMIC - OPERATING CONTROL hazards associated with the plant?

Difficult to understand
Inappropriate coloring
Function not identified
Inappropriate controls & switches
Access and egress
Labelling of controls and indicators
Variation in operators
Operation by two or more persons
CONTROLS - Pre use checks conducted by crane operators. Signage in place in cabin and on crane chassis. Controls to be maintained
-
Are there specific requirements for ISOLATION of energy sources?

Hydraulic pressure
Compressed gases
Electrical feeds/capacitors
Motive power systems
Suspended loads
Operation by two or more persons
CONTROLS - Isolation switch to be turned to off position whenFRANNA is not being used.
-
Does operation of the plant cause extreme TEMPERATURE changes?

Fire
Burns through conduction
Convection
Cryogenic burns
Operation in extreme heat or cold
CONTROLS - Keep clear of hot engine or hydraulic components and exhaust outlet. Allow engine or hydraulic component cool prior to maintenance. Wear appropriate PPE to protect from hot components or fluids.
-
Can a FIRE occur?

Friction
Ingress of materials/fluids
Build-up of materials/lubricants
Fuels
Fire extinguisher
CONTROLS - Dry Chemical powder fire extinguisher affixed within reach of operator (tested and tagged). No Ignition Sources When Fuelling.
-
Can certain WEATHER conditions create a hazard?

Hypothermia / extreme cold
Heat stroke / extreme hot
Wet conditions
Electrical storms
Dirt & mud on roads at egress points
CONTROLS - No works to be completed in extreme wind/rain. Operator to make judgment prior to operations. Only lift when safe to do so. SWMS to be adhered too in regard to weather requirements. Terex operating guide to be followed.
-
Can the plant emit toxic FUMES or VAPOURS?

Exhaust fumes
Chemicals
Haz sub’s/DGs
CONTROLS - If working in poorly ventilated areas, monitor exhaust fumes at regular intervals. Where practical, ensure engine is isolated. Upswept exhaust fitted
-
Carry out the NOISE survey on page 9. Is the plant noisy?

Emit >85 dBA at the operator
Effects operator communication
Noise impacts on community during out-of-hours work (including reversing beepers)
CONTROLS - Persons operating or working near the crane engine bay are to wear appropriate hearing protection. Noise emitted at rear of crane is in excess of 85dBA. Hearing protection must be rated a SLC80 or higher. Isolate the engine when maintenance does not require engine to be running.
-
Poor visibility

At the controls
At the task
Darkens surrounding areas
Light impacts on community or sensitive natural environment during out-of-hours work
CONTROLS - Lighting to be provided if necessary. Ensure all headlights are working.
-
Can operation of the plant create DUST?

Explosive atmosphere
Breathing hazard
Reduced visibility
Nuisance dust at nearby community
CONTROLS - Dust mitigation to be in place to minimise dust level and exposure - Doors and Windows to Franna to be closed at all times when in operation within dusty unsealed roads.
-
Can the plant become UNSTABLE during operation?

Working on uneven / unstable ground
Shifting load
Lack of plant support
CONTROLS - Crane operator to assess ground conditions, load type and load position. Immediatley STOP works should ground conditions become unstable or FRANNA operation on said ground is deemed unsafe.
-
Could LOSS of LOAD occur?

Failure of ropes/slings
Overloading
Entanglement in surrounding structures
Maintenance requirements
CONTROLS - Certified and check equipment to be used. • Cranesafe logbook to be completed prior to use of FRANNA. All questions in prestart to be completed. If any issues are identified on the checklist, they are to be reported to the direct supervisor immediately. • Operator to inspect and check that anti-two block is functioning correctly. Report to direct supervisor immediately if Anti Two Block is not working correctly. • Crane operator to inspect Cranes rope prior to use. Look for any damage. If anything is identified Supervisor to be told immediately and crane not to be used. 3rd Boom Section Does Not Compensate hook block. Operator to be fully aware of this. Rope can be snapped and crane severely damaged if not adhered too. This could result in severe incident.
-
Is there anything in the SURROUNDING ENVIRONMENT that may produce a hazard?

Power lines
Low ceiling
Other plant
Storage areas
Co-located equipment
Isolation requirements
Potential for flash flooding if operating adjacent to waterways
Operating in known areas of weeds, pathogens, or contamination
Operating in sensitive environments requiring protection from offsite weeds/pathogens or spills
CONTROLS - Where crane is driving with loads over uneven terrain, operator may elect to run hook to the boom to prevent swing from occurring to the load and injuring the dogman. This is viewed in the Terex risk assessment as not being recommended however the Terex risk assessment also does not deal with the risk of swinging loads over uneven terrain whilst dogman is in the vicinity. WA Universal see no issue with (two blocking light loads) for the purpose of reducing swing, however this can only be performed when the 3rd boom is fully retracted, Boom stages 1 and 2 are not full extended, the crane is only in 2 falls. And the operator exercises extreme caution. This is not to be allowed for any other activities.
