Fire Protection

  • Fire Extinguishers provided at each stairwell and signage.<br>

  • Standpipe operational.<br>

  • Standpipe connections protected from accidental struck by incidents.<br>

  • Is FDC connection accessible with signage showing its location?<br><br>

  • All combustibles removed from storage areas.

  • Hot work permit received and signed.

  • Fire Extinguisher at each hot work location.

  • Compressed gas gauges operational.

  • Oxy-Acetylene hoses in proper working condition.

  • Compressed gases stored upright, secured, valve closed or capped, and properly stored.

  • Welding leads are spliced with proper plugs, grounded, and in good condition

  • "No Smoking" signs posted at storage areas.

  • Exit signs posted.

  • Exit staircases accessible and free from debris.

  • Evacuation Plan posted

Personal Protective Equipment

  • 100% Head protection provided.

  • Face shields as necessary.

  • Gloves as necessary.

  • Proper eye protection provided.

  • High-visibility clothing worn by all workers and staff on the job site.

  • Proper footwear provided.

  • Respiratory protection provided as required.

Environmental Controls

  • Restrooms Available. (working deck, every other floor, and ground floor)

  • Drinking Water Available per Subcontractor.

  • Dust Control

  • Lunch Area with trash containers cleaned regularly.

  • Mosquito/Pest Control as necessary.

  • Skip pans/Trash bins available for trash disposal.

  • Storm water prevention in place? (Silt fence, storm drains, socks, filter fabrics)

  • Control of any spilled hazardous material (oils, diesel, etc.).

Material Handling/Storage

  • Are all materials, ladders, tools, etc., within 6 feet of the edge tethered and/or tied off?

  • Material stored 6 feet from shaft openings.

  • Materials and Equipment 2 feet away from edge of excavation/trench.

  • Material stored clear of access routes.

  • Materials stacked or secured to prevent tipping.

  • Materials in the laydown yard organized and providing a clear access route through yard.

Rigging

  • All riggers and signal persons have proof of certification.

  • All loads flown with tag line(s).

  • All rigging equipment provided with rated load capacity and identification tags.

  • Outrigger platforms free of debris and chain in place.

Ladders

  • Podium ladders in use.

  • Ladder being used correctly.

  • Extension Ladder - Cleets intact, Secured.

  • Ladder decals legible.

  • Access/Egress - 2 per working deck.

Vehicles & Traffic on Job site

  • Spotters as needed.

  • Back up alarms functioning.

  • MOT Signage in place as per MOT plan.

Tools

  • Power tools provided with proper guarding.<br>

  • Power tools provided with HEPA Vacuum or other Engineering Controls.

  • Powder actuated tool training certificate available for operator.

Scaffolds/Shoring

  • Scaffolds properly braced to prevent tipping.

  • Mud sills/Base Plates/Casters in Place under each leg

  • Cross brace provided at each frame

  • Cross brace lock devices intact.

  • Proper access provided to each platform.

  • Each platform fully decked.

  • Each shoring scaffold properly decked.

  • Scaffold tag in use.

  • Debris netting provided when falling object hazard exists.

Fall Prevention

  • Fall Protection provided at all areas with a fall exposure of 6 feet or more.

  • Workers properly tied off.

  • Lanyards prevent contact with lower level.

  • Personal Fall Arrest anchors rated for 5000 lbs per worker.

  • Lanyard provided limits free fall distance to 6 feet.

  • Harness & Lanyard Inspection Form submitted.

  • Standard guard rails with handrail, midrail, and toeboard installed.

  • Safety Mesh covering railings.

  • Stairwells with proper railings.

  • Hole covers secured to prevent accidental displacement and marked.

  • Drop and outrigger nets free of debris.

  • Controlled Access Zone (CAZ) setup as necessary.

  • Aerial/Scissor Lift Green Tagged.

  • Aerial/Scissor Lift Operator provided certification.

  • Operator wearing fall protection and tied off.

Mobile Cranes

  • Firm ground conditions.

  • Outrigger pads in place and proper dimension and construction.

  • Swing radius protected.

  • Crane Inspections Available.

  • Crane Operator certification available.

Tower Cranes

  • Access to crane enclosure locked.

  • Crane bases clear of debris and water.

  • Electrical panel elevated out of water and locked out.

  • Crossover access deck adequate and safely banded back to slab.

Electrical

  • All electrical panels locked and labeled.

  • Lock Out/Tag Out in place as needed.

  • Properly installed temporary outlets protected by GFCIs.

  • All extension cords without damage and have ground pins.

  • Extension cords properly plugged into GFCI and out of the way.

  • All common areas well illuminated.

  • Properly installed and maintained lighting suspended under ceiling and grounded.

Excavation

  • Utilities located.

  • Excavation Permit received.

  • Support of excavation methods in place, i.e. Benching, shoring, trench box.

  • Proper access/egress provided as necessary.

  • Proper fall protection provided as necessary.

Equipment

  • All operators trained for the use of the equipment with proof of certification.

  • All tires properly inflated.

  • All windows in place surrounding cabin.

  • Operator's manual available.

  • All operator seats provided with seat belt.

  • Fire Extinguisher available on machinery.

Swing Stages

  • Installation and Daily Inspection reports received.

  • Competent Person Identified.

  • Swing covered in mesh and identified.

  • Dampers on safety lines.

  • Power cords and guide wires rolled up as to not touch ground or debris.

  • Anchors visually inspected for deficiencies and reported to relevant party.

  • Green Tagged.
