Type of ladder, scaffold stair access, manlift, or ramp, are they in compliance with known regulations?

Have you spoken with formwork foreman or GC to ensure deck is ready for access by our manpower and preloading of our reinforcing materials?

What is the foreman's (GC) name you spoke to?

Were all questions or concerns addressed by GC & forming contractor?

Rescue plan reviewed with GC? (Man basket & crane or manlift or ramps or other)

Is climbing rebar elements with fall protection? (Additional wall plans, bracing plans, etc. maybe required)

Is vertical work near perimeter i.e., within 10' of deck edge - 100% tie-off required?

Are vertical work plans required? attach plans if required (Tying Procedures, Wall Plans, Column Plans, etc.) If the plan changes, we will need to create a new plan or if new commencement of work.

If formwork is provided, has it been braced to handle rebar, and manpower loads and reviewed with GC and forming contractor? Attach Bracing Plan and pictures of bracing.

If engineered tie-off points are requested and supplied, are they inspected and signed off?

<br>Has all bracing, shoring, and structural supports been visually inspected above and below the area of work being performed? (Refer to engineering drawings/prints with GC or project management team or Superintendent)

As a secondary check we ask that you walk the deck above and below and follow Nucor guidelines for this visual walk through before accessing the manpower or preloading materials in the area?

Are all vertical work plans that are needed completed, approved and signed off? If no, apply any additional work plans here

Fall Protection Plans are required for all vertical work, these are to be reviewed and signed by your supervisor. Site condition adjustment can and should be made by foreman as necessary. *** If you have questions that you are unsure about, contact you're Superintendent or Placing Manager to discuss before accessing deck.
