  • (Victorian Project only need to Draw top view of site plan if damage is found on property). Mark an "X" in the location of the damaged footpath bay and vehicle crossing bay. Draw in the position of the vehicle cross over bay and parking bay if applicable. Photographs must also be supplied of any damage. Please note the number of footpath bays damaged if applicable.
