  • Completing this form please do not forget the following, Please include name of foreman and super. weather examples, hazy, overcast, partly sunny, sunny, open skies, high heat hazard? What is the plan of action for the day? Need an example? ( demolition, grinding old asphalt, stripping, using heavy machinery? please take pictures of barricades, cones, arrow boards, traffic control plans, caution tape usage. Additionally, are flaggers needed? Provide Photo! Is company vehicle parked in a safe location where they can reverse, drive in and out safely? Any obstruction of view? Provide Photo! Fire Extinguishers Must be located by the hygienic white table. It can not be placed behind saws or underneath a seat, even behind another type of tool located in a utility bin. IT MUST BE READILY ACCESSIBLE. Provide Photo! If First Aid Kit is opened they need a new replacement kit. Provide Photo! Check the tag ( does it expire every year or every 2 years? (Is it class ABC classed fire extinguisher?) It should be! Safety Set up section Is it visible to employees located at the job? Do all employee's at this job know where the table is located? Do they know why type of supplies can be found at the table?

Job Site Hazard Board and Safety Station

  • Is the Safety Station Setup and Clearly Visible?

  • Does the table contain all First Aid Supplies?

  • Is the JHA Board Completed?

  • Does the JHA Board List the nearest Medical Facility?

  • Does the JHA Board List job site hazards or risks?

  • Does the JHA Board have emergency numbers displayed?

  • Is the Eye Wash Station visible and available?

  • No first aid contents past their expiry date?

  • Is there easy access to first aid cabinets?

  • Employees aware of location of Safety Station and first aid supplies?

  • Are first aid officers known to workers and accessible?

Personal Protective Equipment (PPE)

  • Are safety vests worn at all times?

  • Is head protection worn correctly and where required by all crew?

Work Site Area

  • Are tools not in use safely stored or placed in secure location?

  • Workers are communicating clearly at acceptable volume (no shouting)?

  • Noise produced by work activities is within acceptable parameters and is not excessive?

  • Does the work areas have adequate lighting?

  • There are no damaged hand tools in use?

  • Is site generally orderly, with trash disposed of correctly?

  • Are trash bags, bins or dumpsters located at suitable points around site?

Demo, Dust and Debris Management

  • Is Demolition or Dust generating activities occurring?

  • Are proper protections to prevent damage from debris in place?

  • Is wet demolition method being used?

  • Is broken material being staged in a designated area for proper removal ?

  • Is the right equipment being used properly for the demolition?

  • Are relevant workers trained in these proper proceedures?

Hand and Power Tools

  • Are all tool guards in place and operable?

  • Are power cords in good condition?

  • Are all tools double insulated or have a ground plug?

  • Are ground fault circuit interrupters in place at the source of electricity?

  • Are portable power tools in good condition?

  • Are all power tools in good condition?

  • No frayed or damaged leads?

  • Are defective pieces of equipment and tools red tagged and removed from service?

  • There are no power across path of travel or walkways?

  • Are tools stored correctly and in the right place when not in use?

  • Are employees using the proper tool for the task they are performing?

  • Are employees using two hand on tools that are designed to do so?

  • Are dust collectors and exhausts provided on tools that produce large amounts of dust?

Powered Equipment & Vehicles

  • Was a walk around check of equipment undertaken before beginning use? Check logs.

  • Does any equipment have warning lights or other indications that it should not be operated?

  • Does the equipment appear to be well maintained?

  • Is any equipment leaking fluids or operating poorly?

  • Are all safety guards in place and in good condition?

  • Is there sufficient room aroud the the equipment to use safely?

  • Are all equipment being operated by trained operators?

  • If training is being conducted, is there visual indication that the operator is undergoing training? (vest/hat/badge or other indicator?)

  • Are all equipment being operated safely?

Incident, Injury & Accident Procedures
