- It was apparent that a new door had been installed into an existing frame. The installation of a new door with associated ironmongery and intumescent/smoke seals into an existing frame may not be suitable in providing the required fire separation as the condition and quality/density of the frame and supporting construction could not be confirmed.
- The door-set only displayed a reference to a fire test and/or assessment and it was not evident that the door-set was of third party independent certification scheme.
- It was not possible to remove door hardware to verify that the correct intumescent gaskets had been installed between the hardware components and the door / door frame.
