  • This inspection form is to be used before any pool vehicles or plant are operated for the day.

  • Instructions:

    1. Start the inspection by taking a photo of the Vehicle Being Inspected.
    2. Answer each item as "Yes", "No", "N/A", "Safe", "At Risk" as required.
    3. Add photos and notes by clicking on the paperclip icon.
    4. To add a Corrective Measure click on the paperclip icon then "Add Action", provide a description, assign to a member, set priority and due date.
    5. Complete audit by providing digital signature.
    6. Share your report by exporting as PDF when access is available.

    Note: Take photos of any noted damage.

VEHICLE EXTERIOR & LIGHTING

  • Are all external lights operational (& Safety Beacon if fitted)?

  • Audible reverse alarm operational (where fitted)? <br>NA if not fitted.

  • Does the Reverse Camera operate correctly?<br>NA if not fitted.

  • Is the ground free of any evidence of oil leaks on ground?

  • If there is evidence of recent fluid leakage then you must check fluid levels and for what is causing the leak.
    If the leak is a large amount then the vehicle is to be tagged out and reported to your supervisor.

VEHICLE EXTERIOR BODY AND GLASS

  • Are all windows free of cracks and chips?

  • Is the body free of damage (dents and scratches) to the vehicle (take photo if unsure if reported already or not)?

  • License plate in position, clean, free of damage (if required)<br>NA if not required.

VEHICLE INTERIOR

  • Are all seats in good condition and have a serviceable restraint attached?

  • Windscreen wipers functioning appropriately?

  • Horn functioning appropriately

  • Air conditioner, heater and demister operational

  • Dashboard lights operational

  • All Gauges are Operatiing and Show within Appropriate Ranges

  • Radio Operational (includes 2 Way)

  • Interior is Clean and Free of Loose Objects

REQUIRED VEHICLE DOCUMENTATION

  • Valid vehicle registration sticker (on trucks and plant)

  • Vehicle is not to be used until confirmation of current registration.

SAFETY EQUIPMENT

  • First Aid Kit available in vehicle (Ensure Completely Stocked)

  • Highway Safety Triangles Available

  • Fire Extinguisher Charged, Inspected and within Test Date?

Periodic Requirements

Weekly Checks to be conducted before use on Monday

  • Check oils and other fluids?

  • Are all tyres in good repair with no visible signs of wear or cracking in sidewall and at an appropriate pressure (includes spare)?

Monthly Checks to be conducted before use on first Monday of the Month

  • Check Fan belts and Hoses?

Sign off

    Signature of all Operators who will be driving over the course of the day
  • Add signature
