1.1 Are floors /walkway surfaces clean & free of cracks, holes, and tripping hazards?

1.2 Aisles clear of clutter?

1.3 Carpets are in good condition and are not damaged or loose?

1.4 Work areas generally clean and organized with good housekeeping?

1.5 Stairwells clear of storage, well lit, and provided with handrails?

1.6 Does the stairwell have a visible sign marked "handrail use"?

1.7 Storage areas adequate for the amount of material being stored?

1.8 Materials stored in a safe and secure manner?

1.9 Is the amount of lighting adequate for the work performed?

1.10 All lamps (bulbs) operable?

1.11 Ceiling tiles all in place and free of stains, holes, cracks, etc.?

1.12 All gas monitoring devices (Oxygen, LEL) up to date with calibration?

1.13 All hand sanitizer dispensers full?

1.14 All machines equipped with appropriate guarding?

1.15 Is unused or damaged equipment disposed of?

1.16 All material handling devices inspected prior to use and utilized by properly trained / licensed individuals?

1.17 Ceiling vents do not contain visible signs of dust or mold?

2.1 All corridors equipped with fire rated doors?

2.2 Are doors held open by mechanical means (no wedges)?

2.3 Emergency egress (exit doors) kept unobstructed with adequate clearance?

2.4 Exit signs fully illuminated?

5.11 Do all dry-ice chests have working hinges (not rusted or compromised) and are free of damage or defects?

5.12 Are snorkel ventilation units inspected and calibrated annually?

6.1 Areas requiring PPE properly labeled with specific requirements?

6.2 PPE (hearing protection, safety glasses, gloves, etc.) available in areas that require them?

6.3 All personnel, contractors, visitors wearing appropriate PPE for the spaces they are working in?

6.4 There is no PPE in office or other eating areas?

6.5 PPE properly stored / disposed of when not in use?
