  • 2.2 Are doors held open by mechanical means (no wedges)?

  • 2.3 Emergency egress (exit doors) kept unobstructed with adequate clearance?

  • 2.4 Exit signs fully illuminated?

  • 2.5 Fire extinguishers secured to the walls at height of 48 inches?

  • 2.6 Fire extinguishers and fire pull stations unobstructed?

  • 2.7 Fire extinguishers fulfilled their monthly and annual inspections?

  • 2.8 Smoke alarms and sprinklers unobstructed with at least 18 inches of clearance?

  • 2.9 Fire strobes and horns unobstructed and easily visible?

3.0 ELECTRICAL

  • 3.1 All electrical wires free of knicks, cuts, and with no exposed wires?

  • 3.2 All electrical outlets are in good condition and have electrical plates?

  • 3.3 Electrical cords coiled when not in use and only used as temporary power?

  • 3.4 Electrical cords not extended along the ground, posing a tripping hazard?

  • 3.5 All electrical panels have external screws / bolts / locks in place?

  • 3.6 All electrical circuit breaker panels / disconnects are unobstructed by at least 36 inches?

  • 3.7 Electrical panels labeled with Arc Flash / Danger stickers?

  • 3.8 All appliances and power tools have 3-prong (grounded) plugs?

  • 3.9 GFCI electrical outlets / cords used when near sources of water or moisture?

4.0 WORK AT HEIGHTS

  • 4.1 All ladders stored in designated areas where they are properly secured and inspected?

  • 4.2 Fall protection (railings, ladder glide devices, etc.) in place where there is potential to fall 4 feet or higher?

  • 4.3 Aerial lifts are inspected and in good condition?

  • 4.4 Elevated areas free of potential drop hazards for people below?

  • 4.5 All non-load bearing areas have appropriate signate?

5.0 CHEMICAL / BIOLOGICAL / COMPRESSED GAS STORAGE

  • 5.1 Areas that contain corrosive materials equipped with emergency eye wash / showers (within 50 feet)?

  • 5.2 Emergency eye washes inspected weekly?

  • 5.3 Emergency shower inspected monthly?

  • 5.4 All chemicals stored in appropriate locations and / or maintained on secondary spill containment?

  • 5.5 All incompatible chemicals stored separately?

  • 5.6 Biohazardous materials / waste properly stored in designated, secured locations?

  • 5.7 GHS labels available for use?

  • 5.8 Adequate containment for broken glass / sharp materials?

  • 5.9 All compressed gases chained upright with valve caps in place?

  • 5.10 The number of compressed gas cylinders is appropriate for the storage devices that are in place to secure them?

  • 5.11 Do all dry-ice chests have working hinges (not rusted or compromised) and are free of damage or defects?

  • 5.12 Are snorkel ventilation units inspected and calibrated annually?

6.0 PERSONAL PROTECTIVE EQUIPMENT (PPE)

  • 6.1 Areas requiring PPE properly labeled with specific requirements?

  • 6.2 PPE (hearing protection, safety glasses, gloves, etc.) available in areas that require them?

  • 6.3 All personnel, contractors, visitors wearing appropriate PPE for the spaces they are working in?

  • 6.4 There is no PPE in office or other eating areas?

  • 6.5 PPE properly stored / disposed of when not in use?

7.0 ADDITIONAL FINDINGS / COMMENTS
