• Risk assessments must be site-specific. If all the risks are covered by Safe Work Method Statements only Risk Assessment Checklist is completed. For any identified risk not covered by a Safe Work Method Statement, a Specific Risk Assessment Form will also need to accompany the Risk Assessment Checklist with the relevant “Risk reduction measure” identified.

• It is the supervisor’s responsibility to ensure that each member of the crew is aware of the identified risks and the Safe Work Method Statements to cover those risks. There is provision for signing off for each member of the crew at the end of the Checklist.
