2.0 EARTHWORKS

2.1 STOCKPILES

  • Stockpiles are at the correct height ratio for soil types / sheer faces knocked down at end of the day

  • Stockpiles are not creating blind spots or hazards to traffic on site

  • Stockpiles to be kept below 2m where possible

  • If the stockpile is over 2m they will be stabilises by hydromulch or other method.

  • If stockpile over 2m where plant is required to travel over or work on top windrows will be built up on outer extremities

  • Where multiple mobile plant and or trucks intersect around stockpiles, directional traffic management has been established

  • Plant movement is uncontested around screens and stockpile locations

2.2 EXCAVATION AND TRENCHING(OSH Regulations 1996 3.109-3.113)

  • Permit to dig in place for area of works

  • Workers signed onto permit to dig

  • Services identified within brownfield permit to dig have been physically identified by pothole methodology or ground penetrating radar

  • External asset authority permits in place

  • Where batter and benching cannot meet the excavation code of practice adequate shoring to be installed

  • Excavation sides battered / benched (horizontal to vertical ratio)dys

  • Trench excavations exceeding a depth of 1m, ladders, ramps or other forms of access and egress are provided at intervals of not more than 30m. Distances of travel for a worker to access / egress is not more than 15m

  • Windrow of adequate size around tag excavation. The windrow will be at least equal to the axel height of the largest unit.

  • Adequate barriers and deep excavation signs around excavation

  • Adequate distance between passing traffic and excavation edge

  • No excavated material or other loads are placed near the excavated area where there is risk that the sides of an excavation may collapse or plant/load may fall into excavated area. Stockpiles at least 600mm away from edge of the excavation

  • Hard hats worn in excavation greater than 1.5m regardless of benching

  • Hard hats worn in any trench at any trench where there is risk of objects falling onto someone in a trench

  • Hardhats worn where there is risk of objects falling onto someone in a trench

  • For excavations greater than 6m the deep excavation checklist is in use (SF 4.25-1.10)

  • Excavations of 1.5m deep there is at least one other person in the vicinity unless the sides of the excavation have been assessed as self supporting

2.3 EARTHWORKS HAULAGE

  • Internal traffic management in place, signage to match risk

  • Haulage route width ample for two way haulage (approx 15m)

  • Haulage route segregated from other works eg) away from workers on the ground and other machines

  • Haulage route free from potholes and rocks

  • Haulage route declines and bends minimised as far as practicable

  • Haulage route two way traffic are adequately separated by windrows where practicable

  • Haulage route edges adjacent excavations are built up and windrows sufficient to axel height

  • No unauthorised vehicles or subcontractors using haul route

  • Giveaway protocol followed

2.4 EXPORT MATERIAL ONTO PUBLIC ROADS

  • Large export jobs have machine with loadrite

  • Verification of load rite data to ensure legal compliance - trucks are not overloaded

  • Export material without load rite - calculation has been conducted for truck configurations and bucket size. Reference export SWMS.

3.0 TRAFFIC MANAGEMENT (PUBLIC AREAS)

  • Works conducted within road reserve or on adjacent public road has an approved TMP?

  • Reference TMP document and TCD

  • Traffic management on site matches TCD

  • Photograph signage and tick off TCD

  • Photograph marked up TCD

  • Traffic management plan is relevant to the stage of works

  • A daily inspection log is conducted by a BWTM (minimum 3 times a day)

  • Signs are legible to road users and in reasonable condition

  • The road speed is reduced to 40km where workers on foot are within 1.2m to the traffic and delineation installed

  • Excavation near public road adequately protected for workers and public vehicles

4.0 PUBLIC SAFETY ( OSH Act 1984 Section 20-22, OSH Regulations 3.17)

  • Site entrance signage in place (if principle contractor)

  • Construction materials kept within the workplace

  • Any risk of objects falling on members of the public

  • Any risk of trip hazards to the public

  • Public access controlled

  • Site perimeter fencing or barricades erected

  • Warning signs in place and relevant to risk

  • Construction signage at interfaces between works and public roads / access

  • Works adjacent public thoroughfare demarcated / barricaded

5.0 OVERHEAD POWERLINES (OSH Regulations 1996 3.64)

  • Electrical clearances over main access roads to job e-mailed to sub contractors

  • Permit to work (RJV)under lines available and implemented. JSA in place

  • Danger zones clearances measured and displayed on signage in the vicinity of powerlines

6.0 RETAINING WALLS

  • Excavations in front of retaining walls are risk assessed and authorised by Project Manager/Structural Engineer (OP 4.18-3.23A)<br>Controls from Engineer/PM are reflected in JSEA.<br>Workers are signed onto JSEA

  • Controls from engineer / PM are in place on ground that is documented in JSA

  • Workers signed into JSEA

  • Fall prevention in place

  • Hard hats worn where any task of wall builders are in line of fire (block delivery or swinging grab)

7.0 SUBCONTRACT MANAGEMENT
