Survival Surgery Observation

Preoperative Preparation:

  • Have the animal(s) been acclimated to the housing facility at least 48 hours prior to the procedure?

  • Has the animal(s) health status been assessed before induction with anesthesia?

  • Is there sufficient supplies of drugs/reagents, sutures, supplies?

  • Are all drugs/reagents, sutures, and supplies within their expiration date?

  • Has sufficient time been scheduled to perform the procedure?

  • Are there any other issue or concerns with preoperative preparation?

  • List issues or concerns:

Surgery Work Area:

  • Is the surgery work space located in the OAR vivarium?

  • Is the surgical work space dedicated to surgery during the procedures?

  • Is the surgical work space located away from traffic, air ducts, and windows?

  • If the surgical work space is a lab bench, are signs posted to warn others not to enter the area during the procedure?

  • Is the surgical area clean and uncluttered?

  • Has the surgical space been disinfected?

  • Have sterile drapes been placed over the surgical work area?

  • Has a sterile field been created for surgical tools and supplies?

  • Are all supplies and instruments ready to use and easily available?

Instrument/Equipment Sterilization:

  • Are instruments cleaned prior to sterilization?

  • What method is us to sterilize equipment and instruments?

  • Are instruments double wrapped?

  • Is an internal integration strip used to verify sterility?

  • Was a temperature sensitive indicator used on the outside of the pack?

  • Internal integration strips are required. External temperature indicators are recommended.

  • Was the pack labeled with the sterilization date?

  • If not used within 24 hours, was the pack stored to prevent exposure to moisture and dust?

  • Were IACUC guidelines followed for length of exposure to chemical sterilants?

  • Describe chemical sterilization process:

  • If multiple animals are undergoing the surgical procedure in one day, how are instruments resterilized in between animals?

  • Are instruments cleaned before being re-sterilized?

Animal Preparation:

  • Is a pre-anesthetic regime required?

  • Describe the pre-anesthetic regime:

  • Is preoperative analgesia given?

  • Is the anesthetic regime adequate for the procedure?

  • Is ophthalmic ointment applied to eyes prior to surgery?

  • Is the surgical site shaved, and the loose hair removed?

  • It's should not be done on the surgical work space.

  • Is the transfer to the surgical work space done sterially?

  • Is a surgical scrub of the incision site performed?

Surgeon Preparation:

  • Is surgical scrubs/gowns/lab coat worn?

  • Is a surgical mask worn?

  • Is a surgical bonnet/cap worn?

  • Are gloves put on sterilely?

  • Did the surgeon (and the assistant of applicable) wash their hands thoroughly before putting on their gloves?

Aseptic Surgical Technique

  • Was the area around the surgical incision site covered with sterile drapes?

  • We're instruments and tools unwrapped without compromising sterility?

  • Did sterile gloves or tools/instruments touch non-sterile items?

  • Were sutures used to close the wound?

  • Were internal sutures required?

  • Were absorbable sutures used?

  • What type of suture pattern was used?

  • Describe other suture pattern:

  • We're external sutures required?

  • Were non-absorbable sutures used?

  • What type of suture pattern was used?

  • Describe the other suture patter used?

  • Was the sterile suture material maintained in the sterile field at all times?

Supportive Care Of Animals During Anesthesia:

  • Is the procedure over 30 minutes?

  • Were body fluids replaced?

  • Is normal body temperature maintained?

Monitoring and Assessment of Anesthesia:

  • Is heart rate monitored and recorded?

  • Is respiratory rate monitored and recorded?

  • Is body temperature measured and recorded?

  • Are hemodynamic parameters assessed and recorded?

  • Mucous membranes should be moist and pink. Capillary refill should be less than 2 seconds.

Anesthetic Recovery:

  • Are animals allowed to recover on a surface free from bedding?

  • Is bedding covered with a drape, and food and water removed from the cage?

  • Are animals monitored until they are awake?

  • Is respiration rate monitored during recovery?

  • Are the animals kept warm?

  • Is the animals hydration assessed?

  • Is the intubation tube removed?

Post-surgical Care:

  • Are animals monitored for pain?

  • Look for changes in posture, appearance, behavior, and/or appetite.

  • Is incision site monitored?

  • For heat around incision site

  • For excessive swelling

  • For purulent discharge

  • Staples/sutures removed 10-14 days post procedure?

Surgical Record Keeping

  • Are surgical records maintained for each animal?

  • Surgical records must include:

  • Date

  • PI Name

  • Surgeon Name

  • AP Number

  • Animal ID

  • Species

  • Weight

  • Sex

  • Age

  • Procedure

  • Pre-anesthetic Agents

  • Anesthetic Agents

  • Time of Induction

  • Time of Recovery

  • Date/Time of Analgesia Administration

  • Date of Suture Removal

  • Monitoring Observations

  • Observations should include date, time, observations, health status

  • Monitoring Schedule

  • Surgical records are considered complete when the post-op regime is finished, and the sutures are removed

  • Every 30 minutes until able to maintain an upright position.

  • Daily for the first 5 days post-op

  • Every third day after the first 5 days until sutures are removed

Evaluation of Surgeon

  • Is the surgeon approved for survival surgery?
