  • What is the requirement for rope access? (Explain task/need for rope access/area/tools/equipment. Has working at height hierarchy of control been considered)?

  • Is a rope access level 3 supervisor, on site, at all times during rope access operations?

  • Does the level 3 rope access supervisor hold a valid IRATA level 3 rope access certification?

  • Does the level 3 rope access supervisor hold an in date first aid qualification?

  • Are all personnel on the team equipped with a life jacket?

  • Is there a sufficient amount of team members, in order to carry out the task?

  • Are the anchor points the suitable size, shape and strength?

  • Is the working design load of the anchor point capable to withstand 15kN (1.5ton) or above? ( Looking for Structural steel / Rated Components etc). No pipework!

  • Is the rope path protected from sharp edges, which may damage/sheer of the ropes?

  • Are all screw gate carabiners set to the closed position?

  • Is the system rigged in such a way, that the team members are connected to, 2 separate points of attachment, at all times?

  • Are ropes connected to the anchor point using IRATA approved knots? (Ask the level 3 to explain what knots he is using, throughout the system).

  • Are there stop knots tied at the bottom of the ropes?
