Rope Access Compliance & Competecy Review

Is a rope access level 3 supervisor, on site, at all times during rope access operations?

Does the level 3 rope access supervisor hold a valid IRATA level 3 rope access certification?

Does the level 3 rope access supervisor hold an in date first aid qualification?

Is the task classed as outboard?

Are all personnel on the team equipped with a life jacket?

Is outboard rescue cover available?

Is there a sufficient amount of team members, in order to carry out the task?

Are all members of the team qualified to IRATA L1 as a minimum?

Have all members of the team had their logbooks checked by the L3 rope access supervisor?

Can all members of the team provide evidence that they have been on the ropes within the last 6 months? (Ask to see Level 1's & Level 2's logbooks).

Has the rope access task been properly planned?

Is a copy of the permit to work available on or near the work site, as per vessel procedure?

Has a toolbox talk been carried out?

Have all risks and hazards been documented on the toolbox talk, and communicated to the members of the team?

Have all tools/equipment been tied off, to avoid dropped objects?

Has a specific rescue plan been carried out?

Are the team aware of what to do in a rescue situation? (quiz the level 1's and level 2's to gain their level of understanding of their role in a rescue situation).

Is the area barriered off, taking into account deflection?

Are the anchor points the suitable size, shape and strength?

Is the working design load of the anchor point capable to withstand 15kN (1.5ton) or above? ( Looking for Structural steel / Rated Components etc). No pipework!

Is the rope path protected from sharp edges, which may damage/sheer of the ropes?

Are all screw gate carabiners set to the closed position?

Is the system rigged in such a way, that the team members are connected to, 2 separate points of attachment, at all times?

Are ropes connected to the anchor point using IRATA approved knots? (Ask the level 3 to explain what knots he is using, throughout the system).

Are there stop knots tied at the bottom of the ropes?
