Flannery Centre Site Induction

  • Has the worker been inducted into the Flannery Centre Safety Procedures?

  • Has the worker contacted the relevant Skillset Manager to discuss work plans?

  • Does the worker have correct personal protective equipment for the task bieng preformed?

  • Has the worker been show the roof access points?

Ladders

  • Is the extentsion ladder rated for industrial use and have a load rating of 120kg?

  • Is it long enough to reach at least 1 metre above the roof line?

  • Will it hook over the ladder access points to stop the ladder from siding?

  • Is the ladder in good condition?

  • Is the ladder set up in at 4:1 ratio?

Safe Use of Ladder

  • Is the ladder on a firm base clear of trip hazards for ease of access?

  • Does the worker understand that tools and materials are to be carried up using tool belt or pouch?

  • Does the worker understand that the ladder is not to be used when slippery conditions prevail i.e. wet, icey or windy?

  • Does the worker understand that three points of contact must be maintained at all times when using a ladder?

  • Does the worker understand that materials must be lifted onto roof using a suitable means? Ropes may be used for light materials.

  • Does the worker understand that the harness must be worn when accessing the roof?

  • Does the worker understand that the lanyard must be hooked onto the anchor point when stepping off or onto the roof?

Safe Work on the Roof

  • Does the worker understand that they are not to work on the roof in slippery conditions and must exit the roof prior to rain?

  • Does the worker understand when working on the roof workers must have the harness attached when working wiithin 2 metres of any potential fall?

  • Does the worker understand that no materials are to be thrown off the roof?

  • Has the worker set up any excusion areas required?

  • Has the worker checked for any other hazards?

If required draw the exclusion zone

  • Draw a plan view

  • Worker completing the work signature

  • Skillset staff member signature
