  • Has a Risk Assessment or SWMS (required for high risk work) been completed and is it being followed? Is there a valid permit signed?

  • Are works being performed in accordance with Permit to Work and SWMS requirements, where applicable?

  • If relevant, does the contractor / partner have in their possession relevant licenses or permits (e.g. hot work, electrical etc)?

  • Do all contractors / partners on site have a valid Working With Children Check?

  • Record the names of all contractors / partners that did not have a valid Working With Children Check in place at the time of the audit.

Safety Awareness

  • Where is your nearest Emergency Evacuation Point? And do you know the La Trobe Emergency contact number?

  • Do you know the nearest medical treatment location should you need medical attention?

  • Who would you call right now if you injured yourself? Name them.

  • Are up-to-date Safety Data Sheets (SDS/MSDS) available for all hazardous substances on the work-site?

  • Do you think your organisation can perform this task in a safer manner than you are now?

Environment

  • Has the contractor / partner erected appropriate safety fencing, barriers and signage?

  • If excavation or landscaping works are being undertaken have the relevant La Trobe underground services and flora maps been reviewed to identify any nearby underground services, sensitive flora (Matted Flax Lilly) or Aboriginal cultural heritage sensitivity areas?

  • Are there any atmospheric hazards i.e. dust, fumes, vapours, radiation?

  • Are waste materials being appropriately disposed/recycled or reused?

  • Is the contractor / partner keeping the work area clean and tidy?

  • Is there evidence of any untreated chemical spills/leaks (Inc. paints/paint clean-up water etc.)?

  • Is there excessive noise being generated which could be seen as causing annoyance, distraction or physical harm to nearby building users?

  • If required has dial before you dig been contacted?

Tools & Equipment

  • Are the contractor's / partner's tools and equipment appropriate for the task? (e.g. metal ladders must not be used for any electrical work) and comply with Australian Standards?

  • Often tools and equipment being used can block the exit route. Is your exit route from when you are to outside clear?

  • Is electrical equipment (including extension cords) tested and tagged showing testing is current, and in good condition? (e.g. look for damaged leads, exposed wires)
