1. Personal Protective Equipment

2. Tools

  • 2.1 All tools have guards and handles in use.

  • 2.3 Compressed gas cylinders not in use have caps in place

  • 2.4 Compressed gas cylinders stored secured & upright

3. Electrical

  • 3.1 Cords are undamaged or properly repaired.

  • 3.2 Cords are protected from damage / hung up

  • 3.3 All tools using GFCI protected outlets.

  • 3.4 Circuits aren't overloaded

  • 3.5 Lockout tagout procedures are being followed

4. Fire Protection

  • 4.1 Flammable liquids are in metal safety cans

  • 4.2 All necessary hot work permits (Sellen and FM Global) are obtained and in place

  • 4.3 Fire watch in place, FE on hand and inspected

  • 4.4 Combustibles removed / protected prior to work

  • 4.5 Person performing work has all PPE (welding shield, face shield, leathers, gloves)

5. Cranes and Forklifts

  • 5.1 Forklift, Crane, Rigger and signal person training current

  • 5.2 Overhead hazards/power lines indentified

  • 5.4 Rigging is inspected for wear and damage

  • 5.3 Tag lines are used during hoisting

  • 5.5 Forklift and Crane inspections completed

6. General Safety Issues

  • 6.1 Required postings are posted on bulletin board

  • 6.2 First aid kit on site and convenient to work area.

  • 6.3 Egresses are clearly marked and utilized

  • 6.4 Public protected: sidewalk unobstructed, overhead hazards, cleanliness, delineation

  • 6.5 Proper signage in place for work being performed

  • 6.6 Nothing Hits The Deck in effect: cleanliness, organization, dunnage vs wheels

7. Fall Protection

  • 7.1 Wall openings & floor holes are guarded or covered

  • 7.2 Guardrails (top 39"-42" and mid 19"-21") and toe boards in place and maintained

  • 7.3 Warning lines are set back from edge at least 15'

  • 7.4 Rebar protection in place for protruding rebar and form stakes

  • 7.5 Fall body harness & lanyard used when employees are exposed a fall hazard

  • 7.6 Fall protection inspected and documented

  • 7.7 100% fall protection in place above 4'

9. Scaffold / Aerial lifts / ladders

  • 9.1 Work platforms are fully planked

  • 9.2. Set up on firm, label surface

  • 9.3 Aerial lifts inspected and used properly

  • 9.4 Scaffolds are plumb, square, in good repair, inspected

  • 9.5 Ladders are in good condition

  • 9.6 Extension ladders tied off top and bottom

  • 9.7 Step ladders are only used in the open position

  • 9.8 Stair or ladder is provided for access points > 19" high

10. Perimeter protection

  • 10.1 Exclusion zone in place below work

  • 10.2 Tool lanyards used where required

  • 10.3 Material secured where required
