Fire Protection

  • Adequate number of fire extinguishers in place?

  • Flammable materials removed from job site properly?

  • Only NFPA approved containers used for fuel?

  • Liquid fuel stored properly?

Ladders

  • Employee using step ladder properly?

  • Are spreader bars locked on step ladders?

  • Are ladders being used free from damage?

  • Do extension or job built ladders extend 36 inches above landing?

Rolling Scaffolding

  • Rolling scaffold wheels locked when used?

  • Rolling scaffold have handrails at 6 feet or above?

  • Outriggers installed if double sacked?

  • Does scaffold have proper access?

Fall protection

  • When above 6 ft some form of fall protection is utilized, either guard rails or harness, lanyard and anchor?

  • Guard rails are at appropriate heights and of sufficient strength?

  • Harness and lanyard are in good shape and safe to use?

  • Proper anchor points are being used?

Review

  • Have all safety concerns not in our scope been brought to controlling entity and/or supervising entities attention?
