A. Employer Docs / Training / Recodkeeping / PPE

  • Is the Health and Safety file on site with all relevant documents?

  • Are copies of the contractor's written Risk Assessment, FPP, JLA's etc. On the job site?

  • Is the daily Toolbox talk logged?

  • Do safety signs comply to the relevant law?

  • Is all the Permits completed correctly?

  • Is there a copy of the PTW Issuer & Holder certificate on site?

  • All workers on site have a valid medical certificate?

  • Are safety meetings documented?

  • Is there a First Aider on site?

  • Are visitor sign in procedures in place?

  • Notification of Construction sent to DOL?

  • Are all registers and checklists completed as required?

  • Are all persons on the jobsite in compliance with minimum PPE requirements?

  • Is the correct PPE used for the task observed?

B. Material handling, Storage & Housekeeping

  • Are loads secured when being moved by means of equipment?

  • Do workers employ safe lifting technique when handling tools and materials?

  • Are materials stored or stacked so as to be stable against sliding, falling or collapse?

  • Are materials stored so as not to present slip, trip, puncture or scrape hazard to workers?

  • Is the site clean and organised?

C. Working at height / Scaffold / Canopy

  • Is there a Scaffold supervisor, inspector and erector appointed with valid certificates?

  • Are scaffold components visibly free of any physical damage or defects?

  • Is scaffold plumb and level?

  • Are Mobile scaffold equipped with locking wheels?

  • If observed while in use, are wheels locked?

  • Does the working platform have a full floor with all the scaffold boards in place?

  • Are toeboards, trap doors and gaurdrails in place on scaffold?

  • Are workers on or near scaffold protected from falling objects if overhead hazards exist?

  • Is ladder provided for access to scaffold work platform?

  • Are work platforms free of slip/trip hazards?

  • Is scaffold erected to SANS 10085 or SANS 51004?

  • Are rooes and buckets used to raise and lower tools?

  • Are all employees hooked up at heights?

  • Is safe access provided to and from roofs or canopies?

  • Fall protection plan with rescue plan in place and followed?

D. Barricading and Warnings

  • Are signs or other warning devices used to inform the public of construction work?

  • Is adjacent public sidewalk and roadway protection provided?

  • Is barricading around scaffold at least 1m from the scaffold?

  • Is a flag person provided to direct traffic?

  • Is the site/work area perimeter fenced or barricaded with a minimum hight 1.2m?

E. Vehicles / Driven Machinery / Mobile Plant

  • Do construction vehicles have functioning back-up alarms or spotters?

  • Are all vehicles used for company purposes roadworthy?

  • Is there adequate seating & seatbelts available for all workers being transported with company vehicles?

  • Does all drivers have valid drivers licences?

  • Are all operators trained, appointed and do they have valid medicals?

  • Is the SWL indicated on MEWP?

F. Ladder

  • Is the proper ladder for the job being used?

  • Are ladders maintained in good condition, joints between steps and side rails tight hardware and fittings securely attached and moveable parts operating freely without binding or undue play?

  • Are ladder rungs and steps free of grease and oil?

  • If any ladders are placed in front of doors opening towards such ladders, is the door blocked open, locked or gaurded?

  • Are ladders set up only on stable and uniform surfaces?

  • Do workers face the ladder when ascending, descending or working?

  • Do workers avoid standing on the top two steps of the ladders?

  • Are ladders placed within safe reach of work?

  • Do workers apply 3 point contact rule?

  • Is there an assistant holding the ladder?

  • Do workers avoid more than one person at a time on ladders?

  • Where ladders are used to gain access to elevated platforms, roofs etc. Does the ladder extend at least 900mm above the elevated surface?

  • Are non-conductive ladders used for work on or near electrical systems?

  • Are extension ladders only used to gain access, not to work from?

G. Fire protection and prevention

  • Are fire prevention and hot work requirements available on site and obeyed?

  • Are fire extinguishers provided and accessible in accordance with the relevant law?

  • Are fire extinguisher inspection tags current?

  • Is a fire marshall appointed and trained?

  • Are emegency plan, assembly points & numbers available on site?

  • Are wet blankets placed over pumps when hot work occurs on island?

  • Are fire watch assigned when Hot work occurs?

  • Are workers protected with PPE when doing hot work?

H. Tools: Hand, Power, Pneumatic and Powder Actuated

  • Do workers position themselves safely when using tools?

  • Do workers avoid exposing others to hazards associated with tool usage?

  • Is work stabilized by appropriate tables, sawhorses, vices or other means?

  • Is cordless battery operated tools in good working order?

  • Are all handtools in good working order?

  • Are safegaurds for tools in place?

  • Are electrical tools grounded or double insulated?

  • Are air compressors equipped with regulators?

  • Is equipment and machinery securely placed to prevent tripping or other movement that could result in personal injury?

  • Do abrasive wheels appear undamaged based on visual inspection and sound test?

I. Electrical

  • Are switch gear, panels and devices that are energised marked and/or gaurded to prevent accidental contact?

  • Are lockout devices available and used on circuits and equipment that could become energised while work is being performed?

  • Is a competent person on site with an understanding of LOTO procedures?

  • Are temporary circuits properly gaurded and grounded?

  • Where observed, no electrical cables used in hazardous zone?

  • Are extension cords in continuous lengths without splice or tape or visible damage?

  • Are extension cords placed so as not to create a tripping hazard or be over-run by equipment?
