The drain cover seal used to prevent pollutants from entering the catch basin (e.g., when fuel is being delivered) is both easily-accessible and does not show signs of needing to be repaired or replaced.

There is an adequate supply of spill response equipment nearby - including<br>an emergency contact list.

The filtration media in this catch basin requires maintenance.

The shut off valve is clearly labeled and accessible.

The UST systems are inspected regularly to ensure no fuel is leaking from the system into the environment.

The facility remains up to date with all requirements set forth by MassDEP's UST Program.

The outfall (DSN-001) is in good condition and is operating normally.

ZONE 2 - BOTTOM WASH SYSTEM & WASTEWATER COLLECTION TRENCH

The Travelift is well-maintained. There is no evidence to suggest there are any hydraulic fluid, lubricant, or fuel leaks.

The wastewater collection system is at (or nearing) capacity.

The filtration media in this catch basin requires maintenance.

Debris being generated during bottom washing is regularly swept up and disposed of properly.

ZONE 5 - NORTHEAST SHORLINE & STORAGE SHEDS

There is scrap metal outside of its designated storage shed (i.e. exposed to the elements).

The floor of the upper shed is free of debris and is scrubbed in between boat<br>moves when possible.

Drums of used oil fuel are stored neatly and labeled according to MassDEP regulations (including dates).

Used oil fuel is transported between the mechanics shop and the upper shed<br>by the pallet (no individual drums).

There is an adequate supply of spill response equipment nearby - including<br>an emergency contact list.

The trench drain (NE corner near the Back 40) is free from obstructions and<br>operating normally.

Ice melt and/or other deicing supplies are being stored outdoors.

All fueling equipment has been properly maintained. There are no signs of<br>leaking, corrosion, excessive wear, or damage.

All fuel dock attendees are adequately trained and know how to respond in the<br>event of a reportable release of fuel/oil into Manchester Harbor.

ZONE 7 - UPPER LOT LEFT

Water is able to flow through the drainage running beneath the commuter<br>rail tracks (i.e., no flooding).

Stored vessels are leaking fuel or lubricants.

Engine zincs have been removed or covered prior to storage.

The filter/sock in the upper lot left is operating as intended and is placed to ensure water will encounter it before passing into the inlet to DSN-004.

No changes have been made to this outfall (DSN-004) by an unauthorized<br>party.

The outfall (DSN-004) is in good condition and is operating normally.
