Exclusion Zones

  • Are there exclusion or restriction zones erected around high risk activities that will prevent unauthorised entry? Including but not limited to, open excavations, holes, skylights, drop zones, well holes etc.

  • Is the fencing/barriers robust enough to prevent entry?

  • Are exclusion or restriction zone signs being displayed on the fencing/entrance gates etc. Red Exclusion, Yellow Restricted.

  • Is the exclusion/restriction zone far enough away from the works so there is no risk to the operatives outside the zone?

  • Locations of the exclusion/restriction zones being communicated in the daily activity briefing or displayed on a site layout plan if there is one? Question - is everyone on site aware of the zones?

  • Is there details of the competent person displayed on the entrance to the exclusion/restricted zone, including name, mobile number and date that zone was implemented?

  • Is there details of the exclusion/restriction zones within the RAMS? Does it state the type of fencing/barriers to be used and how it is to be managed?

  • Are the zones being maintained and checked on a regular basis?

People & Plant Segregation

  • Are vehicles and pedestrians kept safely apart on site using physical barriers where practicable?

  • If so what type of barriers are being used? HERAS fencing, chapter 8, crowd barriers, scaffold A frames or VCBs.

  • Where necessary are there suitable pedestrian crossing places on vehicle routes?

  • Are warning signs being displayed at pedestrian crossing points example 'Warning beware of moving vehicles' ?

  • Do vehicles have good direct visibility or devices for improving vision where reversing can’t be eliminated and where significant risk still remains e.g. external and side mirrors; vision aids such as CCTV; sensing devices?

  • Have pre-use/daily plant inspections been carried out and recorded?

  • Are vision aids clean and are operating correctly?

  • Does the site have a plan showing traffic routes and key controls?

  • Are banksman controlling vehicle and plant movements on site?

  • Are operatives working outside the danger/red zone of the plant/vehicle?

  • Are operatives using the thumbs up industry practice when approaching plant machinery?

  • Is the McGee Thumbs Up Poster on display?

  • Are plant operations under good control and well supervised?

  • Are site operatives approaching from the front in view of the operator and not from the rear?

  • Has people and plant interface been identified as a hazard in the RAMS with adequate mitigation measures?

  • Have the RAMS been communicated to the site team and has it been raised in the daily activity briefing?

  • Have specific Toolbox Talks on Plant/ Pedestrian safety awareness been briefed to the operatives over the past period on project?

  • Are vehicles entering and exiting site under the control of traffic marshals, are barriers being used to close pedestrian paths?

  • Were any Hazard Observation Cards recorded recently concerning the personnel and vehicle traffic or related to unauthorised/accidental pedestrian entry to the plant operating zone?
